Employee was lifting sample bucket around 5lbs out of back of buggy. Causing pain bottom of neck. Employee has worked job at full strength but still felt pain. EE went to clinic and was given restrictions, being put on Workman's Comp starting this week.
QC Lab employee lifted a 5 gallon bucket full of sand and tried to put on an elevated table top. While doing this EE sprained EE's left shoulder causing pain. EE worked with no retractions since 3/26/24. EE had a minor tear to rotator cuff surgery was required Thursday September 19 which led to restrictions and lost time.
Employee was changing bearing at 400 bucket elevator. EE was pushing with a pry bar horizontally slipped and hit left pinky finger on a piece of angle iron fracturing it.
Shoveling filling wheel barrow with sand began to feel pain in neck and pain down left arm. Injury was not reported until 9/19/2023 that is when Comp claim opened.
Employee was working on the air actuated knife gate. The ram that attaches to the gate was disconnected from the gate. The gate was in the down position with the ram being in the up position. EE was spinning the piece that attaches the ram to the gate up the threaded part of the ram and the water pressure coming into the freshwater tank pushed it up into EE's finger.
A dry plant employee had gone to the head box of bucket elevator 175A to clear an obstruction. When completed the employee had leaned the shovel against the hand rail and started to descend down. Upon starting the bucket elevator the shovel had fallen and struck another dry plant employee on the head. This resulted in a light concussion. Dr. had ordered one day off for recovery.
Employee using breaker bar to help customer truck driver close bottom gate. As employee was exerting force employee rolled right ankle resulting in a spiral fracture of the right ankle. Employee was wearing muck boots at the time which did not offer the support needed for job being done.
Employee was using EE's personal knife to cut a zip tie and cut EE's left thumb. EE was not wearing gloves, and was pulling the knife towards EE's body. When EE cut through the zip tie the knife struck EE's left thumb. This resulted in an approximately 1 inch laceration which took 6 stitches to close laceration.
Injury was not reported to company until Tuesday July 27, 2021. Employee was installing a speed bump at the Load Out area. While kneeling down on the right knee EE began to swing hammer side to side to loosen stake to remove from ground. EE allegedly hit left knee with hammer resulting in a cellulitis infection in EE's left knee.
Employee was working on a product spout in silo 1A and attempted to manipulate the limit switch in order to make the spout go down. EE had the guard off of the electric motor and when EE reached in to manipulate the limit switch EE's left forearm had contacted the cooling fans on the rear of the motor. This laceration required 19 staples to close the wound
Employee working on bucket elevator 306 heating a bearing with torch trying to wedge out of place. Employee hit with hammer and was hit by piece of shrapnel on right hand across knuckles causing laceration. Laceration took 7 stitches to close laceration.
Employee slipped while climbing down 10' A frame ladder, EE's left foot slipped off rung resulting in a laceration to EE's back left thigh from a piece of angle iron that was behind EE. Laceration was closed with 21 stitches.
The employee was working as a wet plant operator when a sump started overflowing, the employee was on EE's way to turn off a pump when EE had slipped on the wet concrete. When the employee slipped EE had extended EE's left arm to catch self resulting in a fracture to EE's left arm.
2 miners in a scissor lift tacked a plate, went down to retrieve the welding lead, on the way back up at 16 feet the tacks broke the plate fell 4 feet on ee left arm causing a fracture. Plate was 3 inches by 11 and 3/8 inch. Was tacked at an elevation of 20 feet on 200 dryer. New mine site still in commencing at time of injury.
High Roller Employee was removing a mini refrigerator from back of Ford F-150 pick up and put right hand on bottom of fridge resulting in cutting right thumb. Employee was brought to Winkler County Memorial Hospital and received 10 stitches in right thumb. Employee was released from hospital back to full duty with no work restrictions.
