Mostrando los 6Employee was pulling a 6"x10'L suction hose and tripped over a rock. As employee was falling EE put EE's left hand out to catch self. Employees left hand hit the ground.
Truck Driver in TDW060 pulled over and stopped at a curve in the road, partially off the road, which created a blind spot. Loader Operator in 3LV025 was driving in the same direction of the truck. As the Loader Operator passed the truck the Truck Driver was struck and killed.
Employee was tightening drive belts on crusher using a 3/4ton come-along connected to a 1"x4'nylon strap rated for 6,400lbs. Strap showed no wear prior to use. The strap broke in the middle causing the come-along to go towards the employee and hit right hand. Ice was applied and employee was taken to a medical clinic. X-rays showed that the right thumb was broken.
Slipped climbing down ladder, contusion and sprain right knee.
Working on excavator hydraulic line released pressure and struck EE in left cheek and fluid sprayed in left eye.
