The forklift was being operated by ESI injured employee, on a dry, compacted 3:1 dirt slope. Forklift was traveling backwards slowly down the slope in a zig-zag movement when front tire came off the ground, causing the equipment to rollover twice. Damage to roll-over cab section. Operator had seat-belt on at time of roll over.

Tech was removing a mount plate from a damaged trailer jack stand. The stand sat unsecured on 2 sawhorses requiring stabilization by hand. Using a 4 1/2-in grinder with a cutoff wheel, the wheel moved across a weld, contacting the edge of the material. This caused the grinder to catch and kickback. This force drove the grinder downward into the tech's right thigh above the knee.

While backing from the bank and preparing to load a haul truck, a ~2-ft. rock fell from the loader bucket. The operator did not see the rock, ran over it, and the loader became unstable and overturned on level ground.

Employee tripped over a piece of dunnage under structural steel they were covering with plastic concrete blankets and fell to ground at the same level. The right distal radial bone at the wrist was fractured.

New construction, and addition of a metal flat strap piece was in process of installing. Employee had one end of an 8 inch by 20 ft piece of flat strap clamped and attempted to position the piece in place. The clamp came loose, and the flat metal strap struck EE above EE's left eyebrow. Took employee to the local Dr. Six stitches required and released to full duty.

Employee working on the steel structure of the Sizing Tower tightening bolts on the cross member. The employee was working from a scissor lift to impact bolts with an impact, and sustained debris in their right eye.

Employee had feet slip out from under employee in snowy / icy conditions landing on tail bone area

Slipped in mud; item being carried pinched fingers.

EE bent over slightly to look inside and clean a spring of a drill core barrel. EE was holding a brush (Similar to a toilet brush) that is used to clean out the core barrel. At this point EE's right leg was extended out a little to support themselves,as they bent over. When they stood back up, EE stated that their right knee buckled inward and they fell to the ground.
