Contractor EE was repairing a ripper tooth from a D10 Dozer. EE hit the ripper tooth with a hammer a couple of times to get it in place for the retaining pin. A piece of metal sheared off of the ripper and embedded into the EE's left arm. EE eventually went to the ER for treatment. It was removed and EE was prescribed antibiotics.

EE said the scraper seat was loose all day and jostled EE as EE drove it during EE's entire shift. EE complained of lower back pain.

EE was barring mud out a rail-car when EE slipped and hit the big toe area of EE's right foot on one of the interior ribs of the rail-car. EE broke EE's big toe and bruised the top of EE's foot.
EE was trying to clear an elevator bucket from a screw. Another EE was twisting the drive belt when first EE pulled the bucket out, causing the screw to rotate. The screw hit the first EE's left hand. First EE suffered a broken hand and laceration.
EE was housekeeping below a screw case. EE bend down to clean and hit their head on the frame holding the case up.
EE was removing a pin from the broken bail of a scraper. EE slipped and fell appx eight feet to the ground.
EE was washing out a loader engine deck. EE descended ladder, slipped on the cab step that was covered in wet bentonite and oil, and fell on their right hip/side. EE injured their right hip area and neck.
After rain employee slipped and fell on a wet spot walking to the lab.
Employee was moving screw upstairs when it slipped in hand and pinned thumb back to their wrist.
Employee took guard off to check belt air from outside. The fan kept spinning and the employee put their hand on the belts to stop it, and fingers went between belts and sheave. Employee smashed two fingertips. Employee had gloves on and was locked out.
Employee was trying to dislodge a rock from between discs on tractor and used a hammer and a screwdriver. Employee hit the top to their left hand with the hammer when the rock broke free.
Parked personal vehicle before shift, slipped in mud/fell on backside while grabbing the chock block.
Climbing a ladder and felt a snap in the back of left leg.
Miner was walking into plant when miner slipped and fell onto knee. The following day, the miner experienced swelling and pain. The miner was evaluated by a physician on 5-15-2024 and was advised to rest at home for 2 days while applying ice to injured knee.
Employee was lifting a pipe bender off the ground and felt a pop in the left elbow. EE did not seek immediate medical attention, but the pain didn't go away. Doctors visit was scheduled for 02/01/2024. Employee is now out of work.
Employee was exiting loader, and slipped off the top step falling and injuring their shoulder

Employee was attempting to knock buildup off the inside of a hopper with a hammer, when the hammer bounced back and hit EE in the top of the head.
Employee was attempting to move a large pipe, when they felt a pain in their back running down their leg. Went home and symptoms didn't improve. Employee went to seek medical help on the 8th.
Employee was exiting the loader and missed the last step while descending the stairs, resulting in a hurt ankle. Awaiting x-ray results and employee is off work.
Employee was attempting to close the latch for a tailgate on an end dump truck and as the latch bar moved into place, the palm on the right hand was pinched resulting in a cut needing stitches.
2 employees were barring out a hopper together. One employee was brushing loose material away while the other continued barring and crushed the first employee's left middle finger between the bar and the grizzly, breaking it.
Employee of contractor (truck Driver) suffered a medical emergency (heart attack) while waiting to have EE's truck loaded with bentonite. 911 was called and a ambulance was dispatched to the office of the plant site and the truck driver was taken to the hospital in Worland Wyoming.
at the end of the shift the employee was leaving the plant when they slipped in the mud and fell, hurting their left arm
Employee was barring out a chute, when the door access to the chute swung closed during barring causing the employee to smash their right hand into the door fracturing right ring and pinky fingers.
Employee was cleaning railcars, and when they mounted the stairs to climb onto the car, they felt a pop in their left knee and could not put any weight on it.
Haul Truck operator was hauling dirt to the dump site, when they got too close to the edge of the haul road and ran up on the berm, flipping the bed of the haul truck over onto the road.
Employee was barring down a dryer chute, and upon completion stepped down onto a raised platform. The grating on the platform folded and the employee's leg went through the platform. Injuries suffered were bruising of the shin and a hyper extension of the knee. Medical recommended 3 days of rest and a reevaluation before returning to work.
employee was chipping material buildup inside a dryer hopper when the bar slipped and their left ring finger was broken during the impact between the bar and the side of the hopper.
employees were moving railcars when the employee was told over the radio that the railcars weren't slowing down. The employee jumped on the end railcar and attempted to tighten the hand brake on the lead railcar, but was unable to do so before the front car hit the derailleur and jumped off breaking their left leg.
Employee was cleaning material out of the Mill Hopper when an air cannon purged and shot dust and debris at the employees legs causing bruising and abrasions the front of the thighs and shins.
Employee was ripping rock with a dozer, when the dozer tracked over a rock and dropped. The employee's back started hurting, and they called their supervisor and went home for the day to go see a doctor. Employee has seen a doctor but states they have not gotten their paperwork from the doctor yet 07/25/2022
Employee stepped on the hitch of a fuel tank and hit EE's tailbone on the jack when stepping down. Reported to supervisor and kept working until 6/20/22. Went to Doctor on 6/20/22 and was excused from work for one week after a follow up appointment.
Employee started feeling unwell shortly after arriving on shift. At 11:40pm plant employees took the ill miner to the ER. The hospital could not give a definitive diagnosis and the employee is currently waiting to see a specialist for a proper diagnosis and treatment and is off work at this time.
Employee had improperly adjusted their seat belt, and was driving over rough terrain in the mine site while operating a 627G Scraper. The jarring resulted in the employee bouncing up and striking the right side of their head against the climate controls. The employee suffered a cut on the right side of their head and received stitches at the Medical Center.
employee hit head on inside of cab in a 627G scraper and on 2/25/22 went to clinic to be checked out for concussion like symptoms. employee states they were wearing seatbelt when the rough road caused the contact. the Provider prescribed pain killers and released the employee to return to work on 2/28/2022
Employee was stripping topsoil in a Cat 627G scraper when they over travelled the cut area and drove onto a steeper incline, lower in the pit and the scraper tipped over. Initial 2 visits to doctor cleared the employee for work. Different clinic diagnosed EE with a mild concussion and prescribed a steroid on 11/11/21. Employee will be able to return to work on 11/18/2021.
Employee was using a cutting torch when the torch started to back burn into the barrel. The employee removed the acetylene hose and attempted to clear the torch barrel with oxygen. The mixture inside the barrel was still lit and shot a fireball down the inside of the left glove. This gave the employee 2nd degree and 1st degree burns on the inside of their left palm.
Employee was exiting 627G Scraper when they missed a step while descending. Employee fell backwards and landed on their lower back area and hips.
Employee was attempting to bolt a tire back on a tractor and was stabilizing the tire with left arm, when the tire shifted and jarred shoulder.
Employee was operating a scraper, transferring bentonite and ran over a large rock on the edge of the haul road. The resulting impact from rolling over the rock jarred and strained the employees lower back.
Employee was shoveling dirt when they felt a pop in their back. Reported the injury to the shift supervisor, and informed them at that time they were able to work with no issue. On Sunday 04/18/2021 the employee informed the supervisor that they were experiencing numbness and wanted to go to the doctor on 04/19/2021.
EE was exiting scraper, when the wind caught the door & slammed it on EE's rt hand. Hand was caught between door & door jam. EE continued to work, & on 2/11/21, informed supervisor they wanted to get hand x-rayed. Hand was put in cast & EE given full work release. After working till 2/15/21 EE went back to dr & instructed to stay home till 3/4/21 when injury will be re-evaluated.
Employee stated that they coughed and felt a sharp pain in their groin area, but didn't leave work and stated they felt improvement the next day. Employee ended up filing workers comp paperwork that was accepted on March 3rd 2021.
Employee had completed a welding project on the hopper. The employee was cleaning up the work area and packing tools out of the area. As the emplyee was lifting a 5 gallon bucket full of tools up onto the service cart, they felt a sharp pain in their right shoulder. Employee is going to seek orthopedic examination at this time.
While unclogging a pin mill, the employee accidently stepped on the remote switch lowering the pin mill lid on top of themselves, crushing them between the lid and the pin mill. This resulted in a fractured vertebre and a loss of sensation in the right leg.
