Roof failed at xcut #23 in return on #3 beltline. The fall was a continuation of a previous fall. Unintentional roof fall.

Unintentional roof fall was found at 3:30pm on 4/21/08 at X-Cut #11 on #2 belt #1 entry (intake). Fall occurred in intersection. The roof fall was 4' high, 20' wide and 20' long.

Unintentional roof fall found by on-shift examiner and federal mine inspector at x-cut #14 in #4 entry on #2 belt line. Fall was 6' high, 20' wide and 60' long. Found at 1:30 pm on 3/11/08.

A roof fall in the #2 entry at #101/2 x-cut a wnt to #12 x cut on the #12 belt. It was about 7' to 8" high 20' wide and 140' long.

A roof fall was found to have occurred unintentionally at X-cut 2 to 3 in #5 entry on #3 belt.

A roof fall was foung at #22 X cut in the #4 entry of #2 belt. It was approx 6'-7' high, approx 20'wide and 20' long.

Roof fall at #3 xcut on #4 belt in the #4 entry belt line. It's about 40' x 35' x 7' high. It looks like a large horse back.

A roof fall was found on preshift on # 4 belt it was about 4'or 5'thick, 40'l x 20' w. It had been heaving in the #13 area. It will be set with timber or crib, then start cleaning it up bolting it back.

Hill seam it was just bad top. Will set more cribs so it won't come on any father. it is about 20' w x 40' long x 7' h. It is at @2 xcut on #5 belt in between 2 block between #1 & #2. Found it coming outside at 3:30pm.

Roof Fall is in #11 x cut on #4 belt it is about 6' to 7' high 20' wide and 20' long.

#5 Entry fell in at #5 x-cut in Return. It is about 5 1/2' to 6' high, 20' long & in Entry section 300' inside of mine. Water is coming through the top & it had a hill seam in several places.

A 12'x 12' rock rolled down the mountain and came in contact with the fan and knocked the fan to the side and out of the entry.

Unintentional roof fall occurred at x-cut #13 in entry #3 at face area. Fall covered up the Lee-Norse 265 miner. No injuries to any employees.

He was coming out of mine at end of shift, getting off belt line, tripped, fell onto rollers and hit his knees.

A roof fall was found on preshift at about 7:00 am. It was about 7' thick, 70' long & 20' wide.

Cleaning off the belt line, raised up hit the roof of mine, causing a loose rock to dislodge and fall. It hit him on the left shoulder, knocking him down and falling to floor of mine.

Walking into mines and tripped on water line (hose), causing him to fall.

A roof fall was found at A130 when the #2 belt #2 section belt would not start to put coal on it.

Employee was getting out of the 21 SC shuttle car and twisted his knee:resulting in a sprained knee.

Lifting a box of glue and pulled a muscle in his back.

Unintentional roof fall @ x-cut #23 on #4 belt line intake airway. 8' high, 30' long, 30' wide.

A ROOF FALL WAS FOUND ON PRE-SHIFT AT #33 XCUT IN #2 ENTRY. IT WAS 8 TO 9' HIGHT AND 20' WIDE

UNINTENTIONAL ROOF FALL ON #3 BELT HEADER. THE ROCK WAS SETTING ON CRIB BLOCKS FOR ABOUT 4 YEARS AND IT ROLLED OUT BESIDE THE BELT HEADER AND ON TOP OF IT.

Roof bolting-using a wrench to dislodge a piece of steel let go of it, causing it to hit him on the left ear.

UNINTENTIONAL ROOF FALL ON #4 BELTLINE AT XCUT #28. 60'LONG X 6'HIGH X 20'WIDE.

ROCK OR COAL FELL AND HIT HIM ON THE LEFT SIDE.

Working on a piece of equip, a piece of coal got in eye.

A ROOF FALL WAS FOUND ON PRE-SHIFT AT #29 XCUT IN THE #4 ENTRY. ON FIRST LEFT IT IS ABOUT 10' HIGH 20' WIDE & 45 LONG.

A ROOF FALL WAS FOUND ABOUT 7:00 AM AT #24 X CUT ON FIRST LEFT IN #1 ENTRY THE RETURN. IT IS ABOUT 20' WIDE, 9' TO 10' HIGH, & ABOUT 45' LONG.

ROOF FALL WAS FOUND ABOUT 6:15 AM AT #19 X-CUT ON FIRST LEFT MAIN. IT FELL ACROSS #1 ENTRY, #2 ENTRY, OVER TO #3 ENTRY. IT IS ABOUT 6' TO 9' HIGH & 20' WIDE & ABOUT 110' LONG.

A roof fall was found about 6:30 am at #27 xcut on first left in the return #1 entry. It is about 9-10' high 20' wide and about 40' long.

A ROOF FALL WAS FOUND AT #25 XCUT ON FIRST LEFT MAIN IN THE #5 ENTRY ABOUT 7-8' HIGH AND 65" LONG. IT WAS ABOUT 7:00 AM.

A ROOF FALL WAS FOUND AT #18 XCUT FIRST MAIN ABOUT 20' LONG & 6' HIGH. IT WAS FOUND WHEN I GOT TO #1 TAIL PIECE AT ABOUT 6:30 AM.

SLIPPED AND FELL OFF HI-LIFT.

RAINING WET MUDDY-GETTING OUT OF BACK OF PICK UP TRUCK-MUD ON SHOE AND CAUSE AND HIM TO SLIP AND FALL.

A ROCK FALL WAS FOUND ON EXAM ON MAKING THE RETURN ABOUT 20' BY 75' BY 6' HIGH AT 15 XCUT 15 XCUT AND 14 XCUT. IT WAS FOUND ABOUT 7:30 AM.

ALLEDGED CLAIM-UNDER INVESTIGATION HURT ARM WHILE REMOVING PART FROM EQUIPMENT.

GETTING OFF BELT - HIT ROLLER - BRUISED RIGHT LEG.

ROOF FALL IS AT #17 X CUT IN #3 ENTRY ON THE TAIL PIECE AND ON THE FEEDER IT IS 20' WIDE 6 TO 7 'HIGH AND ABOUT 30 TO 35' LONG

ROOF FALL AT #21 XCUT ABOUT 9' HIGH, 20' WIDE & 60' LONG.

ROOF FALL WAS IN #15 X-CUT IN THE #3 ENTRY ON THE #1 BELT LIME. ABOUT 6'THICK & ABOUT 35-40'LONG ALSO, SAME TYPE FALL IN #4 ENTRY AT X-CUT #17.

WATER CUTTING THROUGH TOP CAUSING ROCK TO FALL AT X-22 #4 ENTRY @ INTERSECTION. 30' LONG 20' WIDE, 9' HIGH. UNINTENTIONAL ROCK FALL.

TRAMMING GRADER BACK TO SHOP ON MOUNTAIN ROAD, IT WENT OFF THE ROAD AND TURNED OVER.

CHANGING A DRILL BIT, HE CAUGHT HIS FINGER BETWEEN BRUSH & SEAL.

TAKING PIPE DOWN, SLIPPED OUT OF HAND AND MASHED RT. 4TH FINGER.

HE WAS HELPING DO MAINT ON EQUIP., SQUATTED DOWN AND THEN COMPLAINED OF KNEE HURTING.

CLEANING UP THE HEADRIVE, FELL.

CAUGHT FINGER BETWEEN BELT CLAMPS.
