REPAIRING TRUCK I.NAME WAS SWITCHING BATTERIES FROM DRAGLINE SHOVEL TO GENERATOR TRUCK

I.NAME WAS GRIN8ING ON DRAGLINE BUCKET HAND SLIPPED AND CUT LEFT INDEX FINGER CUTTING TENDONS AND PART OF BONE

EMPLOYEE WAS REPAIRING EUCLID ENGINE. HE SUSTAINED A LARGE INGUINAL HERNIA WHILE HE WAS TORQUING THE HEAD DOWN ON THE ENGINE.

SHOVEL WAS BEING MOVED TO ANOTHER LOCATION. THE CABLE HAD TO BE REMOVED FROM A DITCH DURING THE PROCESS. EMPLOYEE ATTEMPTED TO PULL THE CABLE OUT. THE PULLING RESULTED IN AN INGUINAL HERNIA FO R EMPLOYEE.

WHILE IN THE PROCESS OF MOVING THE SHOVEL AND ITS ELECTRIC CABLE EMPLOYEE ATTEMPTED TO MOVE THE ELECTRIC BOX WHICH WAS FROZEN IN THE GROUND WHEN HE TRIED TO LIFT IT HE PULLED MUSCLE IN HIS BAC

EMPLOYEE WAS GREASING SHOVEL LOST HIS FOOTING AND BUMPED HIS RIGHT FOOT ON BOOM RESULTING IN BROKEN RIGHT FOOT
