Blaster's Helper was driving/operating Blasting Agent Bulk Truck when he missed a gear descending a 14% grade ramp. Driver was unable to stop or slow truck resulting in truck crashing into highwall. Employee suffered contusion to head and upper body.

Employee was cutting a pump hose and cut his right leg.

Employee was climbing into a dozer. He felt a pull and snap in his right shoulder but didn't report the injury to the office at that time. The injury got worse and he reported it on 8/12/14 after going to the doctor.

Blaster was using a man shovel to shovel drill cutting into bore hole when he felt sharp pain from back to both legs. Results was a strained lower back.

Employee slipped on a rock while working with water pumps

The employee was shutting off a heat valve on a tractor. His right thumb was caught on the valve.

Employee was climbing into a rock truck. He felt a pull on his left leg. He injured his Achilles heel and may require surgery.

Employee was pulling a pump that was submerged underwater. He pulled his elbow and shoulder. He wore a brace but the injury got worse. He went to the doctor on 7/29/13. He reported the injury to the office on 7/30/2013

Employee slipped off a dozer while fueling it and twisted his left ankle.

The employee was in a truck with a crane attached. He was lifting a pump with the crane. The crane fell and jerked the truck up and slammed it down. The employee hurt his back with the impact of the truck when it fell.

EE was working on a dozer with an air wrench. A bolt came loose pinching his middle finger on this right hand.

Employee was setting out pumps. He slipped on a rock and fell.

Employee was getting off of a bulldozer. He twisted his ankle on February 25, 2012. The ankle got worse by Monday February 27. He left work and went to the doctor.

Employee was pulling a track pad off of a dozer. His hand was caught between the master pad and weights. His right index finger was broken.

Loading wet bags by holding/lifting loaded bags with left hand/arm and reaching for empty bags with right hand/arm and strained chest muscle.

Employee stepped on wet bore hole cuttings and slipped straining right hip area.

Lightning struck pattern and initiated seven blast holes.

Employee was dumping rock from his 773B rock truck (#63W1910) when truck overturned landing upside down, causing damage to the rock truck and sending him (via ambulance) to the hospital with a back injury.

Coal truck driver for Black Gold Trucking had got- ten loaded with coal and was leaving out of the coal yard. He got almost to the gate when he died with a heart attack. The truck rolled back down a slight grade and turned over on its left side. Damage to truck 7000.00 approx & 3000.00 approx. to the dump trailer.

Employee was climbing 3 ft. hill when he lost his balance and fell.

EE WAS WELDING FOR ABOUT THIRTY MINUTES. WORKED ENTIRE SHIFT NO PROBLEMS, RIGHT EYE START BURNING ABOUT 2:00 AM NEXT MORNING 10-25-03. STILL DOESN'T KNOW WHAT CAUSED EYE BURN.

HITACHI EXCAVATOR WAS REMOVING OVERBURDEN ON COAL SEAM WHEN IT STRUCK A LOADED DRILL HOLE, CAUSING SAID HOLE TO DETONATE, CAUSING DAMAGE TO DOZER NEARBY. NO INJURIES WERE CAUSED TO OPERATORS.

OPERATOR WAS CLIMBING DOWN OFF OF HIS MACHINE AND FELL TO THE GROUND.

EE WAS MOVING HOSE FROM HOLE (BLAST) TO BLAST HOLE WHEN PAIN WAS FELT IN MIDDLE OF BACK.

EE WAS OPERATING HIS DOZER PUSHING ROCK. A ROCK WENT OVER EDGE OF BLADE AND GOT CAUGHT IN TILT CYLINDER AND LINES. HE PULLED UP TO TOP OF A HILL AND TRIED TO REMOVE ROCK HIMSELF. IN DOING THI S, HE STRAINED HIS BACK.

EE WAS HAVING THE ROCK TRUCK HE WAS OPERATING LOADED WITH COAL BY A FRONT END LOADER. A ROCK FELL OUT OF THE LOADER BUCKET AND HIT THE ROCK TRUCK, JARRING THE ROCK TRUCK AND EE CONTACTED THE FOREMAN AND INFORMED HIM THAT HIS BACK WAS HURTING AND THE FOREMAN TOOK EE TO E.R. TO HAVE HIS BACK EXAMINED.

EE WAS WORKING ON THE CHAIN OF A ROTARY DRILL WHEN THE CHAIN UNEXPECTEDLY MOVED, CAUGHT HIS RIGHT HAND AND ROLLED IT WHICH RESULTD IN THE SMALL FINGER BEING FRACTURED.

EE WAS HELPING LOAD MATERIAL BEING MOVED FROM THE MANCHESTER MINE TO THE NEW MINE IN TUSCALOOSA COUNTY AL. AN EYE WHICH HAD BEEN WELDED ONTO A PIECE OF STEEL BROKE OFF WHILE BEING LOADED ONTO TRUCK. THIS ALLOWED THE SHEET OF METAL TO SLIDE DOWN HIS STOMACH (SCRATCHING & CUTTING) & HIT HIS LEFT THIGH BRUISING & CUTTING IT

EE WAS DISCONNECTING THE PUMP HOSE FROM THE WATER TRK AFTER FILLING TRK W/WATER TO SPRAY ROAD. AS HE REMOVED THE HOSE, DUE TO THE WATER HAVING WET EVERYTHING THE HOSE WAS FALLING FROM HIS GRI P.AS HE JERKED HIS HAND OUT OF THE WAY OF THE HOSE CONNECTOR HE HIT HIS HAND ON A B3OLT ON THE PRAY BAR WHICH RESULTED IN HIS HAND BEING SLICED BETWEEN THUMB & INDEX FINGER REQUIRING SEVERAL S

EE WAS FUELING HIS DOZER AFTER COMPLETING HIS SHIFT. WHEN HE FINISHED, HE STEPPED OFF DOZER INTO A HOLE AND TURNED HIS RIGHT ANKLE OVER. HE WENT TO THE EMERGENCY ROOM AT WALKER BAPTIST FOR T REATMENT. WAS TOLD TO PUT ICE PACK ON IT AND ELEVATE FOOT. MEDICATION FOR PAIN WAS PRESCRIBED. HE WAS OFF WORK THE FOLLOWING DYA, CAME BACK 4-20-00, WAS CALLED BACK TO DR'S OFFICE ON 4-24-0
