EE was checking the hydraulic oil level; he lost his balance and fell from the dozer platform.

Employee bent over, to move a dozer roller, and felt something pull loose in his chest, in the front, on the right side.

Injured employee was operating an excavator on the slate. He swung the bucket around and caught a rock and jerked his lower back.

The driver backed up in stockpile to dump coal and turned over.

Crank handle on spray bar struck right hand, resulting in a cut to the right index finger that required stitches.

Due to flooding conditions due to excess rain the seven foot drain pipe under the mine yard stopped up and water entered the portals. Resulted in water roofing at the #2 belt drive blocking all entries for travel.

Employee was removing bolts from a structure when a piece of draw rock fell hitting him on rt side of neck and shoulder.

EE was getting on a loader to load himself and his hand slipped and he pulled a muscle in his shoulder and elbow.

Employee was building stopping and a piece of draw rock fell hitting him on the right side.

While loading tools on Mac 8, the Mac 8 started to roll and ee hit left elbow on frame of Mac 8.

Victim was dropping miner cable in order to move miner, when cable was suddenly pulled by 3 wheeler, causing cable to bust loose from remaining hangers and strike him, knocking him down.

Roof fall 10-14 ft. high, 18 ft wide, 110 ft long.

An unplanned roof fall occurred in the return at spad No. 451. The fall measured 45' long x 18' wide x 6' thick.

Unplanned roof fall in return entry. Area cribbed and dangered off to prevent entrance by fellow employees. Fall was approximately 18' wide by 30 ' long and 3' to 4' thick.

Victim was tapping in a miner bit insert into the bit lug on the miner, when an object (likely a metal fragment), struck him in the right lower eye lid, then entering the eye. *Note: Victim was wearing eye protection*

Person was putting roof bolts up and got glue in right eye at #4 heading.

Running the loader to work, the road ran into a railroad track that was under the road. The operator's head hit the windshield of the loader and injured his head, neck and back. He was not wearing his seat belt.

Cutting bands off bundle of roof bolts. Band flipped back and cut right wrist.
