Mostrando 50 de 89A roof fall occurred on 6/2/11 at spad # 13 & 30 in the #2 return entry. Fall was 18'W. X 30' L. X 6' H. Did not affect flow of air and will not be cleaned up. No equipment involved.
EE stated he was operating battery scoop hauling mud-muck and loose rock that had been piled up in XC between supply road and belt entries. While backing the scoop a large piece of rock came in operators compartment struck the EE on his left shoulder causing contusion. EE started missing work on 5/27/11.
EE stated he was replacing conveyor chain on a joy 21sc shuttle car using a com-a-long. Chain broke on the com-a-long causing ee to fall backward, causing a possible strain to ee back and neck. EE started missing work on 4/14/11.
Employee stated he was changing insert on drill steel. Roof bolter operator moved the roof bolting machine forward. Right front wheel (tire) ran upon employee's right foot causing a sprain. Started missing work on 3/22/11.
Occurred during the week of 2/10/11 thru 2/17/11. Roof fall was found on the weekly examination by EE and MSHA inspector. High 12ft X Width 20ft X Long 45ft at spad No.4t80 in the main west No. 1 entry. Return air course.
A roof fall occurred in East sub main at spad No. 13+00 in the No.3 entry. 35' long x 20' wide x 6' high. Roof fall won't be rehabilitated. All available entrances to roof fall will be supported according to our roof control plan. No equipment involved. Did not impair ventilation, nor impede travelway.
Employee stated he stepped off roof bolting machine and felt pain in left knee. Started missing work on 4/29/10. Returned to work on 5/3/10.
Employee stated he was changing transmission in a Dapco three wheel rib runner. While lifting up on the transmission, employee stated he felt a sharp pain in his back. Started missing work on 12/16/09.
Employee stated he was working with a crew in the process of moving the mining equipment power cables up two crosscuts. His feet became entangled in a roof bolter machine cable loop causing the employee to fall causing pain to his right side. Employee started missing work on 11-6-09.
EE stated shuttle car anchor came loose that was attached to a bolt anchor to the mine roof. Cable struck EE on his right side which caused a contusion. EE started missing work on 11/05/09 RTW 11/09/09.
Employee stated he was using a belt knife. Knife slipped while cutting curtain causing laceration to right leg. Nine stitches - no lost work days.
EE stated cable broke that was holding up hood on a 10-person alpha mantrip. Struck EE on top of head (left side) (wearing hardhat), causing a possible concussion. EE started missing work on 10/2/09.
EE stated he was connecting a pull rope (wire rope) to cat.245 back hoe. Cut ring finger on right hand. Required 13 stitches. EE returned to work on 8/26/09, did not miss any scheduled work days.
EE stated he was riding in Wallace 14 person man trip. Ran over pot hole in supply road. Causing pain to employee's back. Started missing work on 8/27/09. Scheduled to return to work on 8/31/09.
EE stated he was walking down slope. Slipped and fell, causing pain in lower back. Started missing work on 7/31/09
EE stated he was crossing a belt, hung his right foot on belt rope, causing him to fall off belt. Struck 3" water line, causing pain to his lower back. Started missing time on 7/28/09
Employee stated he was removing a belt "A" frame from mine roof. One end of belt "A" frame dropped down. Other end caught left hand ring finger between roof and "A" frame causing laceration to finger requiring 17 stitches.
Employee stated he was walking under a back-up ventilation curtain, struck his left knee against shuttle car bumper causing his left leg to hyper-extend causing pain to left leg.
A roof fall occurred at spad #4+80 in the no. 3 entry in the first main west supply road....width 20 ft x length 30 ft x high 12 ft.
Slope entry was slick causing mantrip to slide striking steel square set cauisng pain to back and right hip started missing work on 12/11/08.

Employee was using electrical hand held drill, drilling hole into steel lid. Drill bit hung causing drill to rotate, caught employee's right thumb in cord requiring 12 stitches to laceration on thumb. Did not miss any work days.

EE stated he was drilling roof bolt hole in mine roof, piece of draw rock fell from roof, struck EE on his left hand and caused contusion to left hand.
Employee stated he was involved in changing out a pot on a continuous miner, cleaning area where new pot would go. His left hand was caught between pot and frame of continuous miner, causing fracture to middle finger and laceration to ring finger on left hand.
EE stated he removed belt framing from belt trailer. He dropped a piece of belt framing on top of his right foot causing swelling and appearance of a contusion to employee's entire foot. Started missing work on 3/20/08, returned to work on 3/25/08.
Employee stated he stepped off the Cat. 910 forklift onto the diesel scoop. Footing slipped. Fell off top of diesel scoop, approximately 3 1/2 feet high. Dislocated right shoulder.
Employee stated he was operating a shuttle car, struck bump in haul road, felt pain in lower back. Started missing work on 11-5-07.
EE stated he was using a pry bar to change out belt rollers, prying down on belt and felt pain in his lower back, started missing work on 11/3/07.
Employee stated he was aligning the shuttle car cable into the cable reel. Cable jerked employee's right hand into cable reel causing a laceration and fracture to his right hand forefinger. Employee did not miss any time.
EE stated he was helping the mechanic change a hyd. hose on a double boom roof bolter. He raised the boom, struck the bit tray, tray fell, struck employee's left hand between tray and joy stick knob causing laceration to the left forefinger requiring 14 stitches. Did not miss any time.
Employee stated he was hanging the continuous miner cable. felt pain in his back started missing work on 9/8/07
Employee stated he was reaching for a roof bolt and felt pain in his leg leg and knee-He was working on his knee's intalling permanent roof support-missed three work days returned to work on 9/13/07 started missing work on 9/10/07.
Employee stated he was pulling continuous miner cable. Strap being used to pull cable pulled in two. Piece of strap came off cable & struck employee on right hand, near his right thumb causing a laceration requiring seven stitches. Employee returned to work on 8/24/07 - missed one day.
Employee stated he was riding in mantrip & a piece of small black slate fell from mine roof & struck him on his chin causing a small laceration requiring five stitches. Employee did not miss any work.
A roof fall occurred in the 7th south panel at spad No. 2 + 00. Approximately 750 feet long x 20 feet wide x 6 feet high. We moved the unit out of the 7th south panel to the new 6th south panel.
Employee stated he lifted the continuous miner cable & felt pain in his right groin. Operated (surgery) on 8-7-07. Started missing work on 8-7-07.
EE stated he was operating roof bolter machine. EE pushed up-down boom lift lever, boom came down and struck EE on his left leg and knee, causing pain. EE started missing work on 6/17/07, RTW on 7/2/07.
A roof fall occurred in the 1st West sub-panel at spad No. 7 + 00. 110 feet long X 19 feet wide X 5 feet high. Additional roof support: 12 foot cable bolts. Also changed from 5' grouted bolts to 6 foot 3/4" tension re-bar roof bolts.
EE stated he was placing a conveyor belt tail roll in place. Come-a-long chain came loose- caught both hands between tail roller & steel plate. Fracture to little finger left hand, ring finger, r. hand, both fingers required stitches from lacerations. Started missing work 6/8/07.
EE stated a roof bolt broke while being installed into the roof, causing roof bolt plate to come off. Struck EE on his right lower chin, causing laceration requiring 16 stitches. Started missing time 5/31/07-ret'd 6/4/07.

Employee stated he was using a sledge hammer to remove a pin holding boom onto roof bolting machine. A piece of steel from pin struck him in upper left arm. He missed one shift. Returned to work on 5-16-07.
Employee stated he was plastering a concrete block stopping with minova plaster---some plaster splattered striking employee in his left eye causing irriation to his eye.
Roof fall occurred in the east sub panel at spad #8 & 50 in the #6 room....40' long & 19' wide x 5 ft high...no equipment or ventilation affected.
Employee stated while getting out of a 486 S&R scoop operator's compartment, he possibly sprained his right ankle and lower leg. Started missing work on 8/29/06.
Employee stated he was retrieving panel bolts for shuttle car panel lid. The shuttle car panel lid fell on employees right foot, causing a fracture to the employees big toe. EE started missing work on 8/15/06.
