EMPLOYEE WAS MOVING HIS SHUTTLE CAR CABE ANCHOR BY DRAGGING THE CABLE AND INJURED HIS LOWER BACK. THIS HAPPENED ON THHE 005-0, 2 LEFT SECITON.
INJURED WAS DRILLING A ROOF BOLT HOLE ON THE 2 LEFT, 005-0 SECTION, WHEN A HYDRAULIC HOSE BURST AND STRUCK HIM IN THE MOUTH.
EE ANDA COWORKER WERE MANUALLY MOVING A DEWATERING PUMP TO A NEW LOCATION NEAR THE TURKEY MAINS AREA OF THE MINE. AS THE PUMP WAS BEING PULLED OVER SOME ROCK, THE INJURED EE TRIED TO HOLD THE PUMP STEADY BUT IT SLIDE AND CAUGHT HIS RIGHT HAND.
AN UNPLANNED ROOF FALL OCCURED AT SPAD 1774 IN THE TURKEY MAINS AREA OF THE 54INCH BELT. IT MEASURED APPROX. 40'X20'X44" THICK CONSISTING OF SLATE AND SANDSTONE APPROXIMATELY 6 CROSSCUTS INBY 2 LEFT 1 HEADDRIVE.
INJURED WAS APPLYING ROCK DUST TO THE RIBS AND FACE AREA OF 2 LEFT SECTION 00-5. WITNESSES STATE THAT AS HE WALKED AROUND THE EIMCO SCOOP, HE SCREAMED AND FELL TO THE GROUND. HE WAS TRANSPORTE D TO THE SURFACE IN A STUPOR. CAUSE OF ACCIDENT UNKNOWN EXTENT AND OR DAMAGE UNKNOWN. DR RESULTS PENDING. TWO SIMILIAR EPISODES HAVE OCCURED DURING EE.
EMPLOYEE WAS GATERING OLD USED CRIB BLOCKS ON THE 2 LEFT 005-0 SECITON TO SET UNDER THE FEEDER. AS HE WADED THROUGH THE MUD HE STEPPE DON ABLOCK WITH NAIL IN IT PUNCTURIN GHIS RIGHT FOOT.
EMLOYEE SUFFERED A STREE FRACTURE IN THE LEFT FOOT HWNE HE STEPPED FORM TH 2 LEFT 005-0 SECITON TAILPIECE ONTO THE BOTTOM.
EMPLOYEE WAS SPOT BOLTING IN AREA OF ROOF BELOW THE FEEDER ON THE 2 LEFT, 005-0 SECITON, WHEN A PIECE OF ROCK ABOUT 10" SQUARE FELL AND HIT THE EDGE OF HTE CANOPY AND CAME UNDER THE CNAOPY AND STRUCK HIS LEFT ARM INJURING IT.
EMPLOYEE WAS IN A DIESEL MOTOR PARKED AT THE MOUTH OF LEATHERWOOD MAINS SWITCH. EMPLOYEE REPORTED ALL BRAKES WERE SET. AS EMPLOYEE STOOD IN MOTOR DECK THE BRAKES SLIPPED ALLOWING THE DIESEL TO ROLL, CATCHING EMPLOYEE BETWEEN THE ROOF AND TOP OF MOTOR- BRUISING HIM.
EE WAS DRIVING A SHUTTLE CAR ON THE 005-0 2 LEFT SECTION AT THE FEEDER HE GOT OUT TO CHANGE DIRECTION IN DEEP MUCK. AS HE GOT BACK INTO THE CAR, HE FELT A SHARP PAIN AND A TERAING FEELING IN H IS LEFT FOOT. HE FINISHED THE SHIFT EXPERIENCINGLITTLE PAIN. THIS WAS FRIDAY. SATURDAY AND SUNDAY HIS DAYS OFF THE FOOT GOT WORSE. ON MONDAY HE CALLED THE CO TO REPORT THE ACCDIENT AND WENT T
EMLOYEE WAS PICKING UP A BELT KNIFE AND FELT A SHARP PAIN IN HIS LOWER BACK. THIS IS A RE-INJURY.
EE WAS ASSEMBLYING A CONVEYOR CHAIN ON THE 007-0 1 RT. SECTION USING HAMMER. HE STRUCK THE CONVEYOR LINK CAUSING IT TO SPLINTER AND SLIVER OF METAL LODGED IN HIS RIGHT LEG CALF.
EMPLOYEE WAS ROOF BOLTING ON THE 005-0 2 LEFT SECTION. INJURED WAS WAITING FOR BOLTER TO BE MOVED, HIS CO-WORKER HEARD HIM YELL AND WENT TO HIM LYING BESIDE THE BOLTER ON THE RIB. INJURED WA S INCOHERENT BUT CONSCIOUS. HE WAS TRANSPORTED TO THE OUTSIDE WHERE HE RECOVERED BUT COULD NOT REMEMBER WHAT HAD HAPPENED. HE WAS TRANSPORTED TO THE HOSP ND ADMITTED. INJURED HAD A PREVIOUS
EMPLOYEE WAS MOVING TOOLS AND BELTCLAMPS FROM MOTOR TO #2 HEADDRIVE ON THE 005-0, 2 LEFT SECTION. AS EMPLOYEE LIFTED A BELT CLAMP, HE FELT PAIN IN HIS LOWER BACK.
EE WAS RIDING IN THE SECTION MANTRIP AT THE START OF HIS SHIFT WHEN IT COLLIDED WITH A RAIL MOTOR IN A CURVE. BOTH OPERATORS HAD FLAGGED AND SLOWED GOING INTO THE CURBE BUT EACH ASSUMED THE OT HER HAD STOPPED AT THE NEARBY TRACK SWITCH. THEYCOLLIDED WITH MODERATE SPEED CAUSING INJURY TO ONE EMPLOYEE.
EMPLOYEE WAS TRYING TO LIFT A MOTOR COUPLING AND SLIPPED AND FELL ON SNOW AND ICE ON THE SURFACEAREA NEAR THE SHOP.
INJURED AND A CO-WORKER WERE SETTING SPADS IN #5 ENTRY ( 1 RIGHT SECITON, 007-0) SPAD #546. WHILE SXETTING UP TRANSIT, A 580 EIMCO SCOOP RETURNING TO THE SECTION, SWITCHED FROM #5 ENTRY TO #4 ENTRY TO AVOID SOME BAD TOP, THEN SWITCHED BACK TO #5 ONE BREAK BELOW INJURED. THE SCOOP WAS ON 1ST POINT, AND WITH REDUCED VISIBILITY THE DRIVE RNOR HIS PASSENGERS SAW THE RODMAN, AND BUMPE
EMPLOYEE WAS TRYING TO SET A STIFF JACK AT THE BREAKE/FEEDER ON THE 1 RIGHT 007-0 SECITON, WHEN HE FELT PAIN IN HIS LOWER BACK.
EMPLOYEE WAS LIFTING AN OXYGEN CYLINDER INTO A RAIL MOTOR ON THE 007-1, 1 RT. SECTION, WHEN HE MASHED HIS LEFT HAND BETWEEN THE TANK AND THE METAL FRAME OF THE MOTOR.
EMPLOYEE WAS ROOF BOLTING ON THE 005-0, 2 LEFT SECTION, WHEN A PIECE OF ROCK FELL AND STRUCK HIM ON THE RIGHT HAND, INJURING HIS LITTLE FINGER.
EMPLOYEE WAS HELPING LIFT A RUB RAIL FOR THE CONTINUOUS MINER ON THE 005-0,2 LEFT SECTION, WHEN HE INJURED HIS LOWER BACK.
EMPLOYEE WAS DRILLING A ROOF BOLT HOLE, OCCASIONALLY GUIDING THE STEEL WITH HIS LEFT HAND. THE STEEL STRUCK A SOFT SPOT IN THE ROOF AND THE DRILL JERKED UP QUICKLY AND CAUGHT EMPLOYEE'S LEFT HAND BETWEEN THE HEAD AND THE ROOF, LACERATING THE THUMB AND FOREFINGER. REQUIRED SEVEN STITCHESIN THUMB. 005-02 LEFT SECTION.
EMPLOYEE WAS USING THE CONTINUOUS MINER TO CLEAN UP LOOSE COAL IN #1 ENTRY OF THE 1 RIGHT SECTION, 007-0. A PIECE OF ROCK FELL FROM BETWEEN THE RIB & THE FIRST ROW OF ROOF BOLTS, STRIKING HIM IN THE BACK.
EMPLOYEE WAS SHOVELING A COAL SPILL ON THE MOTHER-CONVEYOR WHEN HE EXPERIENCED PAIN IN HIS LOWER BACK.

EMPLOYEE WAS ROOF BOLTING ON THE 2 LEFT SECTION, 005-0, WHEN HE INJURED HIS MIDDLE FINGER ON THE RIGHT HAND. EMPLOYE WAS IN-ATTENTIVE AS TO THE PLACEMENT OF HIS RT. HAND & INADVERTEDLY RELEAS ED THE DRILL HEAD AND CAUGHT THE END OF THE MIDDLE FINGER.
EMPLOYEE WAS FIREBOSSING THE 005-0, 2 LEFT SECTION AND SLIPPED ON SOME MUD COVERED ROCKS. EMPLOYEE ALLEGES THIS RE-INJURED HIS LEFT KNEE.
EMPLOYEE WAS PLASTERING THE RETURN LINE BLOCK BRATTICE ON THE 005-0 2 LEFT SECTION. EMPLOYEE TRIED TO SLING PLASTER INTO SOME DEEP CREVICES AND THE PLASTER HIT THE BRATTICE AND SPLASHED INTO E MPLOYEE'S RIGHT EYE.
EE WAS HELPING LIFT AN OXYGEN CYLINDER ON THE 005-0 2 LEFT SECTION WHEN HE FELT A BURNING IN HIS LOWER BACK. HE FINISHED THE SHIFT, BUT COULD NOT GET OUT OF BED THE NEXT DAY.

EMPLOYEE WAS RIDING A OPEN LOCOMOTIVE TO THE OUTSIDE OF TURKEY NO. 4 MINE. IN AN AREA OF AMPLE CLEARENCE, HE STRAIGHTENED FROM SLUMPED TO A SITTING POSITION, BUT FAILED TO LWOER HIS HEAD WHEN THE CLEARANCE RETURNED TO NORMAL.
EMPL WAS BOLTING TOP ON THE 005-2 LEFT SECTION WHEN A SMALL PIECE OF ROCK FELL & STRUCK EMPL ONTHE RIGHT HAND, CUTTING THE LITTLE FINGER. CUT REQUIRED 3 STITCHES.
EE WAS INSTALLING A PIN IN THE MANTRIP BUS ARM ONTHE SURFACE OF TURKEY NO. 4 MINE THE ARM WAS SUPPORTED WITH A PRY BAR. EE WAS ALIGNING ARM WITH PIN USING HIS FINGERS AND THE PRY BAR SLIPPED, THE ARM FELL AND CRUSHED THE TIP OF EE S LEFT MIDDLE FINGER CHIPPING THE END OF THE BONE.
EE A VISITING FEDERAL COAL MINE INSPECTOR WAS CHECKING THE TORQUE ON A ROOF BOLT ON THE 005-0 SECTION 2 LEFT. AS HE APPLIED PRESSURE TO THE TORQUE WRENCH, HE FELT PAIN IN HIS LOWER LEFT ABDOME N.
EMPLOYEE WAS HANGING MINER CABLE ON THE 2 LEFT, 005-0 SECTION, WHEN HIS LEFT KNEE BECAME DISLOCATED.
EE RODE OUT DIESEL MOTOR BECAUSE MANTRIP WAS INOPERATIVE WHEN THE OPEN DECKED DIESEL WENT THRU A SET OF AIR LOCK DOORS EE THOUGHT HE HAD PULLED HIS ARM BACK ENOUGH TO CLEAR BUT THE ARM CAUGHT ON THE EDGE OF THE DOOR, BRUSING IT.
