While walking down the stairs of the Hitachi 1800 Track Hoe, the injured slipped on the last step and fell backward on the steps straining his back. Missed 3 days, worked 2 days and then missed one more day.

While helping to set a water pump, the injured got his thumb caught in a chain as pressure was applied resulting in six stitches and the end of his thumb fractured.

While helping a mechanic and using a chisel and hammer, the injured hit the chisel and a piece flew off and got in his eye requiring a doctor to remove it.

While dismounting from a highwall drill, the injured's boot lace caught on the steps and he fell to the ground, causing back strain.

While cleaning up the ramp into the pit, a 992 Loader bumped the right rear of a 773 end dump truck parked close to a 2nd end dump facing opposite directions. The truck operators were conversing through open doors and the bumped truck bumped into the 2nd truck causing the open door to slam on the 2nd truck operator's upper arm and caused a strain/contusion to the arm.

While walking across the minesite, the injured slipped on a rock and fell on his rear end causing a bruised tailbone.

Lift cylinder on 773B Rock Truck fell apx 1 foot and pinned left hand to tire.

EE told his wife on phone at 11:30 pm that he was tired & would call her back later. At about 12:00 am, the loader he was operating had a flat tire & he went to get another loader. At about 4:40 am, the ee was found inside the cab of the loader that had not been moved since it was parked earlier on the 26th. Death believed to be "Natural Causes". Autopsy was inconclusive.

Employee cleaning windows of a coal pit truck, slipped and fell landing on his feet and injured his right foot.

The Tipple operator was grinding on parts for the tipple when the grinding wheel he was using fell apart and hit the claimant in the right hand. The claimant was standing 20 feet away at the time.

No unusual conditions, employee instaling cutting edges on D-11 Dozer using 1" Drive Impact over head. He states it caused damage to his neck causing a pinched nerve and numbness. He also had extreme pain in his left arm & shoulder.

WIND BLOWING HARD AND WORKING OVERHEAD ON A D-11 DOZER, BLEW FOREIGN OBJECT (STEEL PIECE) INTO RIGHT EYE.

Was replacing a hydralic hose on a D11 Dozer and stepped from the ripper onto the track. There was a globb of grease as he stepped. He slipped and fell off the dozer. At the time he said his back and hip didn't hurt as bad. It gradually got worse. He worked through December 4th and notified his supervisor on December 6th.

AS THE INJURED DISMOUNTED FROM HIS EQUIPMENT, HE STEPPED ON A ROCK, TWISTING HIS LEG AND HE STRAINED HIS HIP.

WHILE STEAM CLEANING HIS SERVICE TRUCK, THE EE LOST HIS BALANCE AND FELL OFF THE TRUCK TO THE GROUND AND STRAINED HIS ANKLE.

WHILE OPERATING HIS BULL DOZER, THE INJURED SUFFERED A REPEATED TRAMA HIP INJURY STRAIN FROM THE CONSTANT MOVEMENT OF THE MACHINE.

WHILE PUSHING SLOTS WITH HIS DOZER, THE INJURED BACKED OVER A HUMP AND THE DOZER SLAMMED DOWN JARRING & STRAINING HIS TAIL BONE.

WHILE DISMOUNTING FROM A LOADER, THE INJURED SLIPPED OFF THE LADDER AND FELL TO THE GROUND SPRAINING HIS ANKLE AND FRACTURING HIS THUMB.
