A miner was working from a ladder while applying a patch to a screener feed box. While reaching, the ladder slid, causing the miner to fall onto the ladder. The miner sustained a laceration to the right knee and a fracture of the right patella. The miner was prescribed medication and lost time.

The miner did not report the injury to their supervisor until after they went to see a doctor on 04/24/2026. The miner was diagnosed with a left shoulder strain and received work restrictions.
A miner was pulling on a screen in a screener with a metal hook to get the screen over the tension bar, when the hook slipped from the screen. The miner fell backwards and landed on their right elbow causing pain. The miner was seen at an Occupational Health Clinic and received job restrictions.
Maintenance employee was walking in the Breezeway and slipped and fell on some ice, landing on their shoulder. The area was previously clearing of bulk ice accumulation and de icer pellets applied before the slip occurred.
A miner was working at an electrical panel when they attempted to wipe sweat from beneath their safety glasses with their left hand. They did not realize that they were holding a small 14-gauge jumper wire, which contacted the white area of their left eye causing visible redness and slight bleeding. The miner was seen at a clinic and received prescription medication.
On December 28, 2025, Miner was working on the demolition and removal of an existing dryer. Miner attempted to cut an additional rigging point while load was suspended. Miner accidentally cut the supporting rigging causing the load to shift and strike Miner in the abdomen/chest area resulting in a broken rib and internal bruising.
Employee slipped and fell due to icy accumulation at the exit door.
The employee slipped while descending two steps on the north mezzanine floor in Coloring. During the fall, they lost right-hand grip on the handrail, fell backward, and struck the corner of a toe board with the upper left side of their back. The injury resulted in fracture of the left thoracic 8-9 ribs and a bruised lung.
A miner was using a stand-on skid steer to perform housekeeping around a transition point of two outdoor conveyors when they struck their hard hat on the end of a pipe. The miner was taken to the emergency room where they were diagnosed with a head and cervical spine injury and prescribed lost time.
During annual hearing testing the employee experienced a standard threshold shift in the right ear.

EE reports cleaning the bottom floor of the Mogensen building when two palm size rocks fell from the Mogensen screener and hit the EE in the upper back and neck area. EE complains of continuous pain, soreness, and injury.
A miner was performing housekeeping duties by sweeping material between two conveyors into an access hole to a bin. As they backed up and turned to get more material, they stepped into the access hole with their left foot causing their right ankle to bend. The miner was seen at a clinic and given a walking boot and restrictions.
A miner twisted their knee while moving a tank grate from a welding table to a cart. The miner was seen at a local emergency room and received a knee brace and job restrictions.
EE was performing general HK activities dumping wheel barrels in the cooler basement and bumped EE's right shoulder on a conveyor scraper. EE was in contact with the onsite nurse for conservative care. On 6/12/25 EE asked to be sent to Concentra.
EE reports pain and swelling to right knee. EE reports was in the Komatsu truck 845 for 12 hours on Monday, recognized right knee pain and swelling on Tuesday morning upon waking up. Reported on 5/16/25.
EE was operating an electric power pack to remove the roll spring bolts from the roll crusher. EE was using a bolt and a ram to press the bigger bolt free. Once the EE relieved the pressure off the ram the bolt fell and struck the employee's right hand and pinky finger. The EE was wearing gloves during the incident.
Employee was bumping the bottom of tank C1 on the 2nd floor of shipping department and the flop- gate assembly broke, fell off and landed on top of EEs hardhat. EE's hard hat was displaced, and part of the assembly struck the left side of EE's head at the hairline.
While unscrewing a bin-level probe by hand (without using a tool) for maintenance/troubleshooting, the employee felt pain in their right hand. The probe was threaded tightly and the employee applied force as it was not budging. It gave way quickly, resulting in the injury (sprain).
Employee was helping a co-worker lift and place a rotex screen onto a rotex rolling cart. Employee had their right hand between bottom of screen and framework of cart during the lift movement. The employee's right index finger (at fingernail location) was pinched into the frame of the cart.
The employee was placing a hooded sweater into a gang box. When the employee reached inside the lid/door moved forward & down, hitting employee on top of head. Since employee removed hardhat so employee could take off the sweater, it left employee's head exposed. Employee received a 1 inch laceration.
A miner was inspecting a crusher from a platform. When they stepped down to the floor they rolled their left ankle. The miner initially received first aid and on 12/3/24 went to an occupational clinic for medical treatment. The miner was diagnosed with a sprained ankle and received work restrictions.
While ratcheting the come-along used to close a chute the pad-eye/anchor failed and contacted the employee on the head (hardhat), face, and back.
A miner was walking out onto the crusher floor from the control room and realized they forgot their earplugs. They abruptly turned back the opposite direction and as they planted their foot and turned they felt a pop. The miner went to an occupational clinic and was prescribed restrictions.
A 3 person team was removing 11" round metal ductwork for cleaning. A 5' section came free of the quick-camp and dropped down 5' hitting an employee in the metatarsal area of the Left foot resulting a bruise to the foot. Employee was on restrictions, and was released to full duty on 11/11/2024.
A miner was performing a crusher inspection and was in the process of reaching overhead to a work platform to retrieve a tool when they strained their right shoulder. First aid was administered and the miner was sent to an occupational clinic where they received work restrictions.
The employee was walking between 2 conveyors and hit their head/hardhat on a low cross chute. The employee was diagnosed with a neck strain/spasm.

EE was coming down from the 4th floor. Upon descending the steps from the 2nd to 1st floor, EE slipped and injured right knee. Ambulance was called and EE was transported to the hospital.
Employee was attempting to put the hub back on a roll crusher. As they were screwing the hub back onto the shaft, the employee thought the hub had caught the threads. Employee let go to reposition their hands to continue to screw the hub on. At this point, the hub dropped off and smashed the right index finger between the hub and pulley.
Our onsite nurse Identified a potential STS in employee's right ear. The employee will be scheduled for a follow up for verification.
A miner was troubleshooting a crusher oil leak. While leaning over shining a flashlight, the miner felt a sharp pain in their lower back. The miner went to the Emergency Room, was diagnosed with a strain and prescribed restrictions and medication.
A miner was using a wrench to remove a tachometer bolt when they injured their shoulder. They notified their supervisor and received first aid. They continued to self-manage the injury with first aid. On 01/16/2025 they notified their supervisor that pain was persistent and they would like to go to a clinic. The miner received prescription medication and work restrictions.

EE stated due to lifting heavy rotex screens they believe they overexerted themselves and overtime caused a shoulder injury.

Employee was heading home after work and fractured right hand pinky finger while walking up a flight of stairs at the facility. Employee lost footing while walking up the stairs and used right hand to break their fall, resulting in the injury.
Employee was doing a preshift inspection on the sweeper. Employee took the filter out and bent over to tap it on the ground to clean and when the employee stood up, EE felt a pull in their lower back.
Employee reports they were setting up hoses to guzzle in the south mezzanine. Employee experienced a sudden sharp pain in the lower back when bending over to pick up a hose. The injury occurred during the movement/motion of bending, prior to lifting the hose.
It was determine on 07/01/2024 that one of our maintenance employees experienced a right ear STS during our annual hearing conservation testing. The EE didnt miss any work related to the STS.
Employee climbing off of excavator tore bicep tendon on right elbow
Transfer of dust from skin to eye lead to conjunctivitis of the right eye.
As the employee was stepping down from the side of a Rotex screener EE's right foot slipped and EE's foot made contact with a raised portion of the Rotex support leg at floor level which twisted EE's right ankle outward.
Employee was walking down stairs by 7 baghouse magnahelic. They stepped on the second stair they felt pain in foot,left arch.
While shoveling material from around #5 elevator, Employee felt a sharp pain in the lower abdomen area that radiated to EE's groin. Employee then proceeded to see OHN where EE was assessed. OHN then referred EE to local off-site clinic for further evaluation. Employee was seen and released back to work on restricted duty.
A contractor working under USES entered the elevator to proceed to 4th floor. The elevator began going up and suddenly stopped about 3ft off the ground of 1st floor. A plant personnel managed to open the door enough for the employee to squat down and scoot out. Around lunch time the employee reported to the supervisor that employee felt discomfort in the groin area.
EE stated they reinjured their back when they slipped going down to the sub-basement to perform housekeeping activities.
Sandvik 660 on line #3 drive belts caught on fire. At 10:44 PM smoke was observed coming from the floor plates by line #3 cone crusher. We beleive the drive belts were slipping and caused increased friction and heat resluting in them catching on fire.
Employee was closing freight elevator door, their hand slipped off and was caught in between the 2 doors. Pinching fingers on right hand.
