Mostrando los 19The employee violated plant rules by jumping to the ground off a conveyor. EE used their left arm to stabilize the jump. EE's arm stayed on the conveyor when EE landed straining the muscles in EE's arm and shoulder.
Miner was exiting the excavator # 2775 walking southwest towards the haul truck #1517, when miner stepped on a mid-size rock. The rock shifted causing the miner to fall backwards to the ground.
While positioning the edge protector with a hammer a piece of steel splintered off the protector and hit the tech in right upper thigh penetrating skin and imbedding in leg.
On 7/15 operator struck his left elbow on a steel box while sweeping. On 7/27 he reported this injury. He saw a Doctor on 7/28 and was given first aid. The operator visited the doctor on 9/28 and was placed on modified duties. Lacking suitable duties, the Operator was placed on Workers Comp. on 9/30 pending further medical assessment to be conducted on 10/22.
While removing a pin to drop the belly pan on the D-10 dozer, the Miner was using a bar to leverage the pan and remove the pin. When the pin came out, the pan dropped, causing the bar to jerk downwards and smashing the miner's finger against the track. The Miner sustained a cut on the middle finger of his right hand.
While raking waste clay away from a tail pulley the miner struck his elbow on a conveyor support beam. Miner suffered contusion to the elbow. Physical therapy prescribed by P.A. Miner reported injury on 2/6/2015.

While re-installing a machine safety guard associated with a primary clay crusher, a maintenance repairman lost his footing while working off a 41" high elevated surface falling feet first, his back and neck striking nearby surfaces causing injuries.
Employee was walking over to inspect cooler and felt strain in his back. He continued climb a ladder to inspect the unit and condition worsened.
Miner was working in a dusty environment while changing a belt scraper. He complained of dust getting in his eyes. He flushed them with water but the irritation continued. He was examined by a MD; nothing in the eye. The Dr. issued the miner a prescription for medication. The miner was released to full duty at the conclusion of the exam.
Miner was shoveling dirt when a foreign object became lodged in his eye. Ophthalmologist removed the foreign object using a slit lamp. Antibiotic eye drops were prescribed.
In an attempt to wash the front windshield of the front loader, #2094, the employee crawled out onto the front portion of the vehicle not provided with safe access, lost his footing, and fell sideways causing his right chest area to strike the deck handrail.
Employee was sliding grating with a sleaver bar. A section of grating moved and when he stepped on it, it became unstable and he fell through it.
The employee was using the left hand fingers to align a piece of grating suspended from a crane, and an I-Beam. One of the fingers got pinched betwen the grating and the I-beam. The finger sustained a fracture and a contusion.
The employee stated that while prepping a foundation a dust plume came through and dust got into the eye.
Employee hit his foot with a sledge hammer.
FORKLIFT BLADES FELL OVER ONTO EMPLOYEE WHEN HE TRIED TO GET UP FROM THE CRAWLER.
EE REACHED INTO MOVING CONVEYOR BELT TO REMOVE FLANGE. HAND WAS CAUGHT BETWEEN FRAMEWORK AND FLANGE.
