EE was cleaning DGA from under the "Scalping Screen" at the primary plant when EE "rubbed EE's back" against one of the steel cross member bars located at the base of the plant. EE later began to feel pain in EE's back in that area.

Employee is an Equip Opr./Quarry Truck Driver. EE was operating a Quarry Truck, EE was waiting for EE's truck to be loaded when a rock "spilled" or "fell" from the Bucket of the WA-900 Komatsu Mach which was loading EE's truck, broke the driver's side window, hit EE on the L/thigh and fell to the floor of the truck EE was operating.
Employee complained of "lower back pain" after exiting the loader for a mid morning break.
Employee was walking into the "Control Room" when employee tripped and fell on left knee.
Employee was helping change screen panels when they lifted one of the panels they began to feel pain in their groin area.
Employee was changing a V-Belt on under Carriage of the conveyor when EE somehow got EE's "Right Pinky Finger" caught in the area EE was working.
Employee was working on the "Bucket Wheel Assembly" which is part of the Wash Plant located in the BBQ Quarry Yard. EE stuck their finger in the hole where a "bolt" is supposed to go rather than using the proper tool. EE's finger got caught and cut in the process.
Employee complained of back pain which EE felt was caused by the broken seat in the loader EE operates on a daily basis movement of seat started and reported 7/11/2022. Repair & service investigation 7/13/22 parts ordered the same day. Repair work scheduled for 7/18/22 upon receipt of parts, parts delay actual repair date set for 7/20/22 parts are in.
Employee was walking in the primary plant area when they tripped, they did not fall but the awkward step they made to avoid falling caused pain in their right leg groin area,
Employee called plant supervisor complaining of pain in right shoulder due to the use of common tools, ex: hammer, pry bars, etc, while performing maintenance on the screens located in the quarry at the crushing area.
Employee was working with a "PRY BAR" while replacing Screen Cloth at the Secondary Plant. The Pry Bar slipped and employee's L/Pinky finger in the nail area got caught.
Employee was working on crusher chute. The chute has a door which opens and closes for various stone sizes to be directed to. Employee got a pinky finger caught in one of the doors. (Doors are sometimes referred to as gates).
Employee was cleaning the inside of a "CHUTE" when EE began to feel pain in the left side of EE's back and continuing down EE's L/hip & L/leg.
The bucket on the skid steer loader was stuck in the up position. EE was trying to get the bucket to release and rest on the ground. EE was hitting the machine with a hammer. EE got the bucket to release but was standing a little too close to the machine bucket. When it released, the teeth on the bucket scraped EE's left leg in the shin area.
Employee was helping to unload parts from the Low Boy trailer EE had driven when EE stepped off the back of the trailer and fell to the ground. When EE fell, EE hit head on the pavement.
Employee was exiting the Electrical Room located at BBQ Yard, there is one step to ground level, employee missed the step and fell.
EE was using a "pry bar" to loosen screen cloth EE was changing, when EE began to feel pain in lower back.
Employee was cleaning #10 chute. EE noticed a "clump of dirt" falling down toward EE's foot. EE made a "move" to keep it from hitting EE's foot and in doing so, EE twisted ankle.
Employee was using a torch to cut a section from a steel plate when something blew in right eye. EE reported the irritation to supervisor, ee did not feel it needed medical attention just first aid. Overnight the condition got worse, next day ee R/eye was observed to be red and painful. EE was sent to Concentra for treatment.
EE was holding onto an area where a "Nut" was attached with a wrench while another employee was using another tool a "Power Wrench" to loosen the nut. The vibration from the "Power Wrench" caused ee to loose grip, the wrench ee was holding onto "slipped" and the result was a "Fractured finger".
Employee was shoveling stone around #7 screen when ee felt pain in back.
Technician was in the process of repairing a broken lift cylinder mounting bracket on a Caterpillar model 777D Truck. As EE was attempting to align or pull together the mount and the frame with the portable hydraulic press in order to weld the broken bracket, the tools on/off button failed causing the c-clamp to pop off striking/lacerating the area above right eye.
Employee was stepping off a portable ladder EE was using to power wash equipment, when EE somehow fell and hurt wrist.
Employee was using a utility knife and did not position self correctly when using knife. Employee did not use proper procedure when cutting with keeping EE's wrist and arms tight to reduce being struck by.
Employee was helping to set a crusher bowl in place, one of the cables which they were using to support the crusher bowl broke and hit him in the back of the head and neck area.
Employee was climbing down from the HP 300 Feeder, he stepped onto the HP 300 V-belt Guard, he slipped and hit his right side against the crusher. He was wearing his "Safety Harness."
Employee was using a "portable stair rack" so he could work on a crusher bowl. When he finished climbing the stair rack he said he felt pain in his right knee.

Employee was working on a piece of steel plate. He and a co-worker were aligning the plate. Employee stuck his finger inside on of the holes, the plate shifted and caught his index finger, left hand.
The Shuttle Belt and the HP 300 crusher discharge belt were not de-energized, and the electrical disconnects were not "locked-out" prior to work being performed. See citation #879638
Employee was checking the tire pressure on the Extec screener, when he bent down he cut his face on a piece of metal on the machine. RX= Tetanus Shot.
EE was watering the quarry roads. As he was driving down on of the roads he tried to use the brakes to slow down he says the brakes "failed" and he was unable to stop. He managed to turn the wheels towards the embankment was of the quarry and was able to stop the truck that way.
Employee tripped over a piece of wood and sprained his ankle.
Employee was using a shovel to clean area under the HP Crusher Plant he says he fell backwards and bumped his R/arm against an I Beam which is located at the plant. On May 9, 2013 he came to work and reported he had swelling of the R/hand and forearm due to the bump he experienced 2 days prior.
Employee was using a "hand held" welding torch to cut "U bolt" from the equipment. He failed to turn the torch "OFF" while he moved himself to a better position to complete the work. When he reached for the torch, he burned his hand.
Employee was cleaning ice from the WA 480 Loader, when he slipped and fell while trying to close the door.
EE was working with another employee to remove a "steel cover plate" as they were lifting the plate it slipped and hit the EE on the left thigh.
EE and co worker were replacing a "Jaw Crusher Maganese Bolt". EE was using a "Sledge Hammer" and bar to loosen the bolt, he missed hitting the bar and the hammer motion took him, and the hammer into a steel plate which is part of the jaw. He was still holding on to the hammer handle and his fingers got caught between the plate and handle.
Employee was removing a plate from the Magnet on C-10 Conveyor when the plate "bounced upwards" and hit his Left Thumb. Note: RX Prescribed & therapy.
EE was cleaning around #7 screen area. He was shoveling and hand picking stone when somehow a stone "bounced upward" and hit him in the face. He sustained small cuts and bruises on his nose and forehead. Note: X-rays taken and RX prescribed.
Employee was climbing down the steps of the wheel loader he had been operating when he began to feel pain in his lower back area.
EE was installing a "rear end" on a pickup truck, when foreign matter got in his right eye.

Employee was lifting a pump when he began to feel pain in his back.
Employee was cleaning the shop area with a broom when he began to feel pain in the muscles of his upper back.
Employee was operating a "articulate truck", he was "dumping a load" when it. "jolted", he said he began to feel pain in his lower back. He stated he had previously injured his back, and was being treated by a chiropractor.
