EE was operating the shuttle wagon while loading cement rail cars. EE had EE's head out the window, watching for signals from the ground man. A gust of wind, blew some debris up and under EE's safety glasses and into EE's right eye.

Checking sheeting on roof to see if placed right to screw down, there was condensation on roof sheeting and EE started sliding off the roof and landed on EE's feet.

Miner cut their left pinky finger when they struck it with a hammer installing traffic signs in the plant. Miner was sent to urgent care where they were given stitches and placed on restricted duty.

EE was inserting a keeper pin into the reclaimer sprocket, to prevent the sprocket from motion, so the necessary repairs could be made. As the pin was being inserted, the sprocket rolled, causing the pin to thrust upwards. Pinching the EE's left index finger between the pin and the guard for the reclaimer sprocket.

EE was closing the door to the passenger elevator. EE pulled the rope to close the door to the elevator, the door came down faster than expected and smashed EE's left hand between the top and bottom doors. Left hand smashed. X-ray revealed 2 broken bones in EE's hand.

Miner was performing work on a 770G rigid frame water truck when EE walked into the water fill pipe on the haul truck cutting EE's right eyebrow. Miner was sent to urgent care where EE received 5 stitches for the cut.

EE was hitting a stuck drill head hammer with a pin hammer and a brass hammer. A piece of the drill head hammer metal fractured off, and entered EE's left leg on EE's inner thigh between EE's knee and groin. Doctor's wanted to wait a few days before performing the removal of the metal, in order to have a good OR room for the removal.

EE was helping to put parts in a crate and the part was set down on a sling. One employee lifted the part by the sling to move it off of the sling. At the same time EE reached down to move the part off of the sling. The part pinched EE's right pinky finger between the wood block and the part resulting in the smashed finger tip.

Originally went to ER. Was released to return to work. After follow-up with ortho DR. was placed on restricted duty on 11/06/24. EE was walking to their work truck when they tripped on some uneven concrete and fell. Injuring their right shoulder.

Miner was using a sledge hammer to remove a stuck rock from the grizzly bars. Upon the rock coming free, a piece of the rock struck miner in the mouth resulting in a fractured #9 false tooth. Miner is still working and having dental corrections made to the tooth.

EE was closing the doors on the passenger elevator and got right hand and middle finger caught between the doors. EE reported the incident on 9/18 as a first aid. On 09/23 EE requested to see a Dr. EE got an exam and X-ray. X-ray was negative. On 09/27 EE went for follow up and was placed on restricted duty because finger looked infected and was given an antibiotic.

EE was conducting a preventative maintenance check on some belt lines. EE reported a trough roller had a hole in it. EE said EE used a pry bar to check for the hole in the roller. During this check, EE's sleeve was unbuttoned, and EE's sleeve was caught by the roller and belt and pulled EE's arm between the conveyor belt and a trough roller. EE pulled pull cord to free self

EE had a STS in their left ear

EE was given an annual audiogram. The first one EE agreed to have done since 2011. EE was reported to have an STS in right ear.

EE was given an annual hearing test. EE had chosen to not have a hearing test conducted since 2009. EE was reported with an STS in both ears. Previous hearing test results do not seem accurate from 2009.

EE had an annual hearing test done. EE has chosen to not take a hearing test since 2009. EE test results showed a reportable STS in EE's left ear.

EE was given an annual hearing test. The EE was reported to have an STS of 24.00 in EE's left ear. Baseline audiogram is questionable.

Miner sprain right wrist while using a swinging sledge hammer during a crusher liner change.

Miner stepped out of operator's station of Cat 14H blade to close open engine bay door. In doing so miner stepped onto rear tire that was covered in mud, slipping & falling to the ground. This resulted in spraining left wrist. Miner had MRI completed on 3/10/24 & results were provided to SRMG safety on 3/12/24 stating there was in fact a fracture in the wrist. No other issues.

EE and another employee were replacing cover grates in the CC-404. They were using grinders with cut off wheels. The 2 employee's were working close together, and the EE's right forearm was struck by the other employee's grinder wheel while it was still spinning, causing a cut on EE's forearm.

EE was walking routine for Preventative maintenance. EE came down a flight of stairs. At the bottom, EE turned to the right and started walking. EE stepped on an uneven joint in the concrete and twisted EE's left ankle.

EE was inspecting a chute and gate area. EE entered the gate area, and the gate cycled. EE was pinned between the gate and the chute. EE sustained some crushing injuries. The employee sustained a fractured rib.

The EE was standing next to the cart located at the bottom of the preheater tower, having a conversation with the Manager. EE was wearing eye protection, but a gust of wind blew calciner dust off the deck above the EE, and into EE's right eye. EE reported it as a first aid, but the next morning due to irritation, went to the Dr. EE was prescribed anti-inflammatory eye drops.

EE was inspecting RM-306(raw mill) roller deck. EE tripped on a support bracket for the lubrication system lines. The tripping motion caused the EE to fall forward into the rocker arm assembly, causing EE's right arm to strain backwards causing shoulder pain. Originally reported as a first aid, but due to continued discomfort medical attention was sought on 5/10/23.

EE was stepping down the metal stairs from bucket wheel on reclaimer. EE went to turn to walk the to the ladder, the EE's left knee gave out and caused EE sharp pain in EE's knee. On 3/20/23 EE said knee was still sore, and a little swollen. EE went to Doctor, and returned to duty with restrictions. No kneeling, squatting, climbing.

Miner was trying to remove a rock that was stuck underneath the jaw crusher with a 5 lb sledgehammer. While trying to dislodge the rock that was stuck, the miner strained EE's right shoulder.

Miner was shoveling material around the crusher area. After getting home, EE's back became sore and tight.

Miner was running for cover from the starting rain storm when EE stepped on the concrete near the flocculent tank. In doing so EE stepped on a small amount of spilled wet flock and slipped. Miner said they felt a pain in lower back.

EE reported on 1/16 that EE hit EE's head on a beam, the EE was wearing a hardhat. Reported as a first aid only. On 1/17 EE requested to be evaluated, because of pain. Diagnosed with a neck strain and released to work with restrictions. EE was released for full duty on 01/21.

EE was an over the road truck driver for a trucking company, not a miner. EE passed away from as yet undetermined personal medical factors while in the truck cab on the scale at the Camp Verde Gyp Mine. The death was not related to any mining activity, event, incident or action. EE's last CDL medical clearance to drive was on 01/31/2022 and did not expire until 01/31/2023.

On the morning of 12/6/2022 miner reported to their supervisor that on 12/05/2022 they had jumped from the Jaw crusher frame rail and injured their right knee upon landing on uneven surface. Employee was sent for medical and placed on restricted duty for a sprain knee.

Miner was stepping out of service truck when they missed the bottom step and fell to the ground. Miner impacted the ground with left shoulder resulting in an injury to the shoulder.

Cleaning area with forklift, employee attempted to lift forks over the divider, forks were not able to lift high enough resulting in the fork to drop back down smashing employee's left ring finger.

Working in electrical mcc room on a ladder felt knee pop

Incident on 16SEPT2022 was originally a first aid only/no medical case after a 35lb. jack hammer went thru some loose material pulling down on EE's r-shoulder. On 19OCT2022 EE requested medical evaluation because EE still had pain. EE was medically evaluated and released to work the same day (19OCT2022) with restrictions awaiting follow up appointment.

Miner was using a swinging sledge to tighten the mantel nut on the crusher. When the sledge swung past the wrench the miner attempted to catch the sledge on its backswing and got finger smashed between the sledge and the wrench. Miner received medical attention for the injuries and stitches for the cuts.

Miner's personal pocket knife came open in miner's pocket and tore through the material of miner's jeans exposing the blade. Upon sitting down the miner ran hand down leg contacting the exposed blade and cutting hand. 3 stitches were given by medical to close the wound.

Miner was climbing into service truck when they felt something shift in lower back. Miner requested medical attention and EMS was called. Miner was taken by ambulance to the ER where they were admitted for lower back strain.

Miner was adding engine oil to a 775G haulage truck. As EE was finishing the task, EE closed the engine compartment access hood/door. EE wasn't aware that EE's right thumb was placed on a pinch point area and struck EE's right thumb causing a laceration. EE was treated and received stitches. Miner was given restricted duty.

EE felt a pop and pain in right ankle when standing up from desk. Suspects related to a F/A only on the same ankle on 04/11/22. Requested medical evaluation. EE was diagnosed with a strained calf muscle and ankle ligament and returned to work with a brace and restrictions. Treatment is on going.

Miner was climbing the ladder of a 775 haul truck during servicing of equipment. While climbing miner felt a pull in lower left abdomen. Upon medical review miner was given restricted duty for a strain.

Miner was changing guarding on crusher when miner stepped backwards and stepped on another guard lying on the ground. This resulted in miner twisting an ankle. Miner was placed in a removable boot from medical and placed off work until Jan 18th, 2022.

Miner was cutting urethane crown bar protectors to size with a utility knife. On the final cut the urethane bar shifted and caused the left index finger to move into the blade's path. The knife trespassed the leather glove and into the side of the left index finger. The miner received 12 stitches.

EE reported pulling a muscle in EE's left upper arm on 05DEC21 while removing a tail pulley guard. Incident was initially a first aid only/ no medical case. On 21DEC21 EE requested medical evaluation because EE's arm still hurt. EE was medically evaluated and diagnosed with a strain of the left deltoid muscle. EE was prescribed P/T and released to work with restrictions on 21DEC21.

Miner was operating a front end loader and sustained a contusion of lower back when the seat of the loader bottomed out when miner hit a bump. Miner was placed on restricted duty upon medical review of the injury.

EE Was cleaning under mill rolls in preparation to set shims when dust fell from above and got behind EE's safety glasses into EE's right eye. EE flushed EE's eye on site but still had discomfort so EE was taken to be evaluated by the eye Dr. EE was treated, prescribed eye drops and released to work with no restrictions.

Miner was inspecting the head pully of a conveyor belt on the Jaw crusher while standing on the chassis of the jaw. Miner stepped over to one of the tires and it rotated causing miner to start to fall. Miner grabbed the support leg of the conveyer to stop the fall and strained right shoulder and upper arm. Miner was placed on restricted duty by medical.

EE was climbing down the ladder on the side of the scrap steel dumpster and over stretched getting to the trailer deck. EE was medically diagnosed with a groin strain and is being monitored by the Dr. for further evaluation/diagnosis.

Miner cut EE's left ring finger while changing segment plates from a John Deere 944K loader bucket requiring 8 stitches. Miner was holding bolt head as EE impacted the nut with the impact wrench spinning the head of the bolt.

While miner was greasing mobile equipment miner went to step back into service truck and felt a pop in lower back. Miner was sent to medical for evaluation as miner requested and was placed on restricted duty due to a reported strain of muscles in lower back by medical.
