Emp.stepped into pump pad flooded with caustic which filled his boots. Went unreported for 2.5 hours. Once notified, supervisor contacted med dept,RN assessed and determined first aid case. Rechecked on 12/1/13, employee off 12/2 & 12/3, rechecked on 12/4 with no signs of infection. Evaluated by Dr. on 12/9/13, & med prescribed for cellulites, resulting in recordable.

GRM grinding in the north door of machine shop got foreign body blown into left eye. Flushed in medical dept by RN, then seen by Dr. and antibiotic drops prescribed. Returned to work with no restrictions. Rechecked & released by another Dr. on 11/11/13.

Employee was walking from Powerhouse jobsite to field maintenance shop on windy day with wind gusts up to 24 mph, when he noticed an irritated sensation in his right eye. Eye flushed by RN prior to sending to Dr., then referred to another Dr. He reported wearing safety glasses. Antibiotic drops ordered for four days. No restrictions.

Employee was setting wooden forms and struck his thumb with a hammer.

Employee reported he was on the topside of 350 (hot end) taking down scaffolding and while untightening knuckle(scaffold clamp) wrench slipped off nut, resulting in injury to left shoulder muscle.

Employee was leaving carpenter shop on bicycle traveling short distance to lunch room for afternoon snack prior to end of work shift. After traveling a short time, front tire of bicycle hit loose gravel. The front wheel turned abruptly, resulting in employee falling to his right side. In the process of attempting to break fall, he twisted right knee.

Employee was removing 4 inch mist eliminator piping on #1 flash tank with mono goggles, but no face shield. Caustic liquor trapped in line splashed on left side of body as he attempted to exit. Showered on site for 10 minutes, then in dispensary for 30 more minutes. Eye irrigated for 20 minutes. Prescription medication dispensed two days later for eye irritation.

Emp. had unreported injury after working on 02/10/2013. He called medical department on afternoon of 02/12/2013 to report tearing in left eye. He was seen by one Dr. to remove foreign body and by another Dr. to remove rust ring & foreign material. Antibiotic eye drops given.

Emp. entered north area of tripper against instructions. Supervisor energized tripper, and tripper struck emp., knocking emp. out of tripper area onto catwalk grating. Coworker hit emergency stop cord, stopping tripper. Emp. reported incident to medical, did not complain of any symptoms for 5 days, then on 6th day, ER visit confirmed 3 compressed fractures to thoracic area.

Employee reported he was using a rod to clean around a conveyor belt. Rod bumped conveyor belt, jamming employee's fingers between rod and structure, resulting in laceration to middle finger and fractures to tips of middle and ring finger.

Employee reported removing 12" pancake blind rom tee on #19 precipitator, when caustic sprayed in his direction. He fell backwards while trying to get out of the line breaking operation,resulting in a right-sided lower back strain.

Emp. was unplugging#2 catchall hopper with lance. While performing,the hose came off spraying hot condensate all over the back of the his legs. He stopped working,and went to check his legs.He told his Supervisor what had happened saying he was ok, and went back to work. On 12-1-12 the medical department was notified of the incident,and RN started treatment immediately.

Employee reported welding on the press floor and when he finished the weld, he lifted his hood, and when attempting to readjust fogged goggles foreign matter brushed against the inner aspect of the outer corner of the left eye.

Employee reported working in Area 335 under bank #2 installing jumper line. When he had to change welding rod, he lifted his hood, when foreign matter fell behind his safety glasses into his right eye.

Emp was prying open 2 flanges to install valve. Utilizing a 3/4 ton come-along & a one inch nylon strap to open flanges. One inch strap broke, causing flanges to slam together with left thumb located in pinch point. One inch nylon strap was not large enough to withstand tension, which resulted in strap breaking. Negative x-ray on 8/22. Nail repositioning done on 8/24/12.

Employee reported being at mud lake #4 at 8:30 am on 7/18 when he was attempting to stand an 8 inch PVC pipe (Pipe weight= 50 lbs) upright, when his left foot slipped backwards, resulting in a pull in the left groin area. Result after examination by Dr. was a left inguinal hernia.

Employee was working in hot temperature and went too long without hydrating self adequately.

Employee reported he was attempting to open valve in 2-tank Geho area when valve handle slipped/spun causing right middle finger to be pinched between wrench & valve body.

Nurse called to lab and informed employee on bathroom floor for an undetermined amount of time, when she called supervisor for assistance, as she reported attempting to lift self off toilet, and resume standing position, when she felt bilateral legs tingling, she then stood up, twisted left ankle, resulting in fall to ground.

Employee was installing metal on an insulated pipe at ground level. There was a valve handle on the pipe, in off position. As EE was working in area, he snagged his tool pouch on valve and pulled into on position, releasing steam and suffering burns.

Employee reports hosing pad in 6th tank area when he got caustic on R wrist. RN questioned R wrist when observed on 12/21/11. Stated he washed area well in work area. Medical dept notified by employee several days later.

EE reported shoveling mud on top of reclaimer tank when he was splashed by overflow line from mud troughs that were emptying into tanks.

Employee was putting on a #2 secondary charge when he reached up to put on injector to vessel a foreign body hit him in face. Went to control room, removed goggles and something fell in left eye (foreign material)

While assisting welder felt irritation right eye at 11 a.m. informed supervisor at 2p.m. break as redness increased. Seen by nurse practitioner at Dr seen by Dr. to remove foreign matter from right eye.

The employee states he was opening a 2" valve and was sprayed with 50% caustic.

While the employee was trying to cut a hydraulic hose with a die grinder that had a disc blade on it, he cut his finger.

The employee stated he was unhooking a hose from #42 overflow pump and was sprayed in the face with caustic.

The employee states he was lifting a valve and tripped and fell on his knee. This was originally a minor injury but his knee has worsened and the employee is going to have surgery.

The employee was swinging a maul at a valve and missed. His neck and scapula starting hurting later in the day. This injury was originally a minor. The injury was not healing so the Doctor sent the employee to physical theraphy making this injury a medical and reportable.

The employee stated that he was opening the main drain valve on the sprayer line and was sprayed with 280 degree liquor. A smaller drain valve on the side of the line was already open and the employee did not know.

The employee stated that he was opening a drain valve and caustic ran down in his boot. This injury was minor then it worsened and now is a LTA.

The employee stated he was using a long rod to rod out the bauxite in the day bin and felt a pull in his back.

The employee stated he does not know when or where he had gotten caustic on him.

The employee was swinging a maul and missed what he was swinging at. The employee hit his hand on a valve and fractured it.

The employee was walking by the lime tank and dust blew in his eye.

The employee was racking in a 4160v tie breaker that was difficult to crank. The employee stated he felt a pop in his lower back that caused his arms to tingle and his back to hurt.

The employee was washing down his area and when he stepped back he tripped over the hose he was using. The employee fell onto a staircase located behind him hurting his shoulder.

The employee stated he was closing a vent valve on #7 heater and pressurized liquid came out of the valve spraying caustic in his eyes and on his body.

The employee stated he was buffing a pump housing and some foreign metal flew in his eye.

The employee was swinging a maul and missed what he was swinging at. The employee stated he felt a pain in the middle of his back. This injury was originally a minor then the employee received a prescription for medicine. This made it a reportable to MSHA.

The employee was walking out of the control room on the top side of 345 and was stung by something. The employee had an allergic reaction and was given perscription medication for the bite.

The employee was leaving area 340 after repairing the holding tank level indicator and walked into a spray coming from the tank. The spray was hot green liquor. The employee said he could not see because his goggles were fogged up due to the steam.

The employee was swinging a maul and twisted his knee. The employee was beating on a valve that he was re-building.

EE was cleaning a scale basket in area 325. He was not wearing goggles and caustic scale flew up into his eye.

EE felt something bite him. The bite area later started swelling and EE reported to medical.

THE EMPLOYEE WAS SWINGING A MAUL BEATING ON A VALVE. THIS INJURY IS A LOST TIME ACCIDENT.

The employee was running the bobcat and the machine started to malfunction. The bobcat started jerking around which caused the operator to be jerked around injuring his back. This injury is a L.T.A.

The employee was stepping off of a stair case and sprained his ankle.

THE EMPLOYEE WAS OPENING A CONDENSATE VALVE AND HOT WATER SPLASHED ON HIM. THE VALVE WAS LOCATED ON THE BACK OF PROCESS HEATER.

THE EMPLOYEE WAS SWINGING A KNOCKER BAR AND FELT A PAIN IN HIS WRIST.
