Employee was cutting a belt with a utility knife, and it slipped cutting EE's arm just above the wrist.

Employee was shoveling out the tail pulley on the under jaw conveyor. When EE was trying to push material out from under the conveyor it caught the shovel therefore twisting the shovel out from EE's hand causing it to rip off half of the left hand index finger nail.

Installing a tail pulley on fines stacker conveyor. Lifted pulley in an awkward position, felt the strain.

Employee was changing the tail pulley bearings on the sand conveyor tail pulley when the cable come along broke lifting the belt roll forward pinching his right hand.

Welder was welding on the crusher in a squatted position. Other potential contributing factors may be heat and lack of hydration. Welder strained his lower back.

EE was cleaning up material by jaw crusher with skid steer loader. He raised bucket to dump material when a rock rolled off the backside of bucket falling into the cab and hitting ee left foot.

As per employee: At 9:30 AM, I employee decided to climb the Ortner to clean it. Then the alarm went off and I jumped off, landed on my left foot and the ground was frozen, and I know I should have not been there, but I wanted to clean it out.

EE was stepping up into a belt to work on it when he lost his footing. He used his right hand to catch his fall when he smashed his right middle finger onto a rock. The rock caused a small laceration to the finger.

EE WAS WORKING ON A DIESEL ENGINE INSTALLING AN INJECTOR PUMP ON A POWER SCREENING PLANT. HE HADREMOVED THE RADIATOR FAN SHROUD, WAS PRIMING & TESTING ENGINE, HAS #1 INJECTOR LOOSE, ENGINE STA RTED & HE WENT TO TIGHTEN UP #1 INJECTOR, & STUCK RIGHT HAND INTO FAN BLADE! RECIEVED SEVER LACERATIONS ON R HAND & WRIST. NO PROPERTY DAMAGE.

WAS CHANGING OUT A HEAD PULLEY BEARING ON #1 FEED APRON REPLACING THE BEARIN. WE WERE DRIVING IT BACK ON WITH A HAMMER WHEN IT SHATTERED & A PIECE OF IT HIT ME IN THE CHEST.

EMPLOYEE WAS CUTTING BEARINGS OUT OF #2 SCREEN WHEN HOT SLAG FELL DOWN HIS GLOVE AND BURNED HIS INDEX FINGER. MINISTERED FIRST AID AND WENT BACKTO WORK.

LEG OF CONVEYOR FELL & HIT EMPLOYEE ON HEAD

WHILE REPLACING LOADER BUCKET TOOTH, EMPLOYEE WAS DRIVING TOOTH BACK INTO SHANK WITH A HAMMER, PIECE OF TOOTH BROKE OFF AND LODGED IN RIGHT UPPER CHEST.

EMPLOYEE WAS WALKING @#3 LEACH DUMP & STEPPED ON THE DRISCO PIPE & SLIPPED LANDING ON HIS BACK.

TWISTED KNEE ON FEEDER AT CRUSHER.

EMPLOYEE WAS UP ON BASE COURSE STACKER CLEANING A PILE UP OUT & SLIPPED & FELL ABOUT 14' TO GROUND.

EMPL CLIMBED DOWN STAIRS OF LOADER.SLIPPED OFF 2ND TOP STAIR AND FELL TO GROUND. LANDED WITH LEFT ARM THROWN BACK AND LANDED ON HAND TURNED BACKWARD.

HUNG FOOT IN SAFETY CABLE. HE SLIPPED AND FELL ON LEFT ANKLE.

TRYING TO PRIME PUMP STARTED TO SLIP TRIED TO CATCH SELF TWISTED BACK

HE WAS TRYING TO BREAL LOOSE A ROCK FEOM A FEDDER BIN AND 50' WHEEL CONVEYOR.
