Miner was operating a haul truck and bringing sand to the underground mine. The haul truck struck the side rib of the decline shaft. The miner suffered rib fractures as a result of the impact.

Mostrando los 14EE and another ee saw big rock in pile that shouldn't be there. We're inspecting screen panel condition & discharge cluck flop gate position. Both men were on opposite sides of catwalk. EE put his hand inside chute and the other ee pulled a lever on the other side. The gate was closed on ee thumb. He received 5 stitches on his thumb and lost his nail.
Employee was docking the dredge for the winter when one of the winch cables got caught underneath the corner of the dredge. Lenny tried to pull the cable free when he strained his shoulder. He didn't think anything of it until weeks later when the pain continued. He went to the doctor on 11-9-2012 and at that point was restricted from work.
A non-employee unlawfully gained access to Reliable Materials Lyons, LLC's private property and apparently committed suicide by leaping from a high wall. The body was found by a Lyons, Illinois police officer. MSHA employees were on the site for an annual inspection and were notified immediately.
Miner was researching part for part number and part came undone from hose and sprayed miner with 10-W hydraulic oil. Miner flushed immediately with saline and as a precaution was taken to a local medical facility. No damage and miner was released to full duty immediately.
Employee was replacing the blade on the d6 dozer when he bumped the hydraulic cylinder with his shoulder causing the cylinder to fall on and cut his left thumb. He severed 2 tendons and required surgery to repair tendons. Follow up appt. is 4/11/08 followed by physical therapy. Released for light duty w/restrictions.
Operating a haul truck down hill of haul road that was iced over, slid & lost control running into embankment. Was not wearing seat belt & he was tossed around the inside of the cab, resulting in fractured left clavicle & possible shoulder separation.
Operator was servicing his crane. He bent over & slightly twisted his body while trying to lift a 5 gallon oil container. He strained his lower back.
PERFORMING ROUTINE MAINTENANCE ON CEDAR-RAPIDS CRUSHER, BUMPED RIGHT ELBOW ON STEEL FRAMING.
EE WAS REMOVING SWING CLUTCHES FROM CRANE. ONE OF CLUTCH HALVES BECAME STUCK. HE REPOSITIONED HAND TO LOOSEN. CLUTCH HALF LOOSENED BEFORE HAND WAS REMOVED, THEREFORE CRUSHING FINGER.
TWO EMPLOYEES WERE REPAIRING GUARD RAIL FOR MATERIAL SCALE. AS ONE WAS PUSHING THE OTHER WAS PULLING AND LET GO, THEREFORE HITTING HIMSELF WITH I-BEAM.
EE WAS REMOVING THE CUTTING EDGE FROM AN END LOADER. HE REMOVED THE FINAL SCREW & MISJUDGED WHERE HE WAS AND DROPPED THE PLATE ON HIS RIGHT FOOT.
REPETITIOUS BEATING WITH SLEDGEHAMMER ON BACKHOE.
EMPLOYEE WAS WALKING WHEN HE SLIPPED AND HIT HISKNEE ON A CONVEYER.
