Was lifting wear parts in and out of a VSI crusher when he felt a strain in the mid section He did not think much about it at the time.

EE was doing clean up under equipment with Bobcat skidsteer loader and got dust/dirt in his right eye. EE was wearing safety goggles.

While cleaning front of windshield on a Volvo 220 loader, ee got dizzy and fell off the loader to the ground. EE was taken to our local medical office and was told he had a broken foot.

While moving the Finlay at the pit to a new section at the time of turning it a rock fell off the ledge of the feeder. The rock was 3" in diameter and hit him on the right shoulder. He rec. medical attention and Tramadol and Ibuprofen was prescribed.

EE's lower back started hurting due to rock truck he was operating had no absorption or cushion in the seat. He hit a pothole and went up and down and felt something stretch in his back. He rec. medical attention.

Lifting plate on USI, plate slipped, fell and cut middle and ring finger on right hand.

While removing tail pulley on a conveyor, it dropped pinching his left palm of hand. The laceration required 5 stitches and medical attention.

INJURED WAS OPERATING IN SERVICE TRUCK AT THE CRUSHER. HE HAD PARKED BEHIND THE WATER TRUCK AT THE CRUSHER SITE. A PICK UP DRIVEN BY ANOTHER EMPLOYEE WAS PARKED BEHIND THE SERVICE TRUCK. EMPLO YEE OPERATING BACKHOE AT THE CRUSHER MOTIONED FOR THE WATER TRUCK TO PULL FORWARD AT THE TIME INJURED PUT HIS TRUCK IN REVERSE AND BACK INTO THE PICK-UP DRIVEN BY EMPLOYEE.

WAS STIRRING CHEMICALS AT SAND PLANT WITH PRESSURE HOSE. HOSE BACK PRESSURED AND WHIPPED AROUND AND HIT EMPLOYEE IN THE FACIAL AREA. KNOCKED HIM DOWN ON LEFT ARM. RECEIVED BROKEN ARM, BRUISED RIBS, FACIAL BRUISES AND TEETH KNOCKED IN.

INJURED WAS ON THE TAIL PULLEY OF STACKER BELT CHECKING GEAR BOX ON SCREW WHEN HE LOST HIS FOOTING & FELL TO THE GROUND APPROX 4-5FT

EMPLOYEE WAS MANUALLY REMOVING LARGE ROCKS FROM CRUSHER FEEDER BELT. ROCK ROLLED OFF BELT ONTO EMPLOYEE'S RIGHT FOOT, FRACTURING THE BIG TOE. EMPLOYEE DID NOT HAVE A STEEL TOE SHOE, AS REQIR ED.

EMPLOYEE WAS PUTTING A BELT UNDER THE CRUSHER CHUTE, HE ATTEMPTED TO CUT A HOLE IN THE BELT WITH HIS POCKET KNIFE. POCKET KNIFE FOLDED, CUTTING HIS RIGHT INDEX FINGER.

EMPLOYEE WAS WORKING AT CRUSHER SITE WHEN A ROCK BECAME STUCK IN THE BIN, HE ROLLED THE ROCK OUT OF THE WAY AND STRAINED HIS LOWER LEFT BACK.

BUSTING ROCKS WITH A SLEDGE HAMMER PIECE OF ROCK CHIPPED OFF & FLEW INTO RT EYE DID NOT LODGE IN EYE BUT DID CAUSE IRRITATION & A PRESCRIPTION FOR AN ANTIBOTIC WAS ISSUED BY PHYSICIAN

EMPLOYEE WAS USING A JACKHAMMER TO REMOVE OLD CONCRETE. AIR HOSE CAMBE LOOSE FROM COUPLING. HOSE HIT EMPLOYEE IN MOUTH CAUSING A CUT IN THE UPPER LIP. THIS ACCIDENT OCCURRED AT INTERNATIONA L MINE CORP. (I.M.C.C.).
