EE walked through water puddles not realizing there was ice underneath. EE slipped. EE reported EE's knee hurt the next day and we sent EE to the doctor who released EE to full duty with follow up visit & therapy. Knee strain was the diagnosis. EE has always been released to full duty but was referred to an Ortho doctor who thinks EE needs to have EE's knee repaired.

Mostrando los 9Stepping down off a tail pulley guard that was 18" off the ground. Guard was covered with rubber belting and had loose rocks on it. As EE stepped down the rock slipped and made EE slide down the side of the rock. EE caught self and this resulted in a dislocated right shoulder. It was an 18" slip
Moving Material to the pep plant got the truck stuck. Went to get out of the truck to see how stuck it was and missed the last step. Fell about 1' landing on back. Reported pain on left side as they took deep breaths or coughed. Sent for evaluation. Initial report was contusion but follow-up next day was broken ribs on the left side.

Coming down off the loader that feeds the second screen stepped on the conveyor didn't see the roller and caused employee to roll his ankle
Removing head pulley from conveyor - chain came off and smashed fingers between the head pulley and metal bar. Fractures/cut to fingers 3 & 4 on left hand requiring stitches.
EXPERIENCED BACK PAIN GOING DOWN THE STEPS AT DESOTO PIT LOCATION SCALE HOUSE IMMEDIATELY AFTER CLOCKING OUT. IT IS OUR BEST GUESS THE INJURY OCCURRED EARLIER WHILE SHOVELING.
WHILE CUTTING CHAIN LOOSE WITH CUTTING TORCH, CHAIN CAME LOOSE AND CAUGHT HIS HEAD, PINNING HIM. HE HAD TO BE CUT FREE.
WHILE DOING DREDGE REPAIR, EMPLOYEE PULLED WRENCH UNDER OBJECT WITH SLAG EDGES WHICH CUT INTO TOP OF RIGHT HAND.
