Technician was removing a 29.5X25 tire from the right rear position, after the tire came off the wheel, tech was setting the tire on the ground which cause the chain to loosen causing the tire to dislodge and strike the technician and caused EE to loose EE's balance and fall to the ground, landing on remote control strapped to EE's shoulders causing the fractures to the pelvis.

Employee was climbing down the ladder of the 980G front end loader when his foot slipped, due to muddy boots, and fell against the upper handrails, cracking a rib on his left side.

Worker was assisting in removing a metal pole from the ground by wrapping a chain around the pole and attaching the opposite end of the chain to the skid steer bucket. The chain slipped from the pole causing the bucket to lunge upwards, where the worker had his hand, crushing his hand in between the under side of the lab shack and the bucket of the skid steer.

Loading a chute on a trailer with a loader and the chute swung catching the employee shoulder to shoulder between the chute and trailer frame, causing some bruising and sprain to shoulders.

Cleaning inlet chute on rock crusher. Moving parts struck the bar used for cleaning and threw it back out striking palm of right hand.

Wrench fell from catwalk striking employee on forehead.

Welding slag, partical in left eye from grinding.

Hammering pins in shanks for loader bucket teeth & hit right thumb with a hammer. Split open end of right thumb

HELPING CHANGE SCREENS ON A SCREENING PLANT & THE WRENCH SLIPPED CATCHING HIS RT. HAND, LITTLE FINGER BETWEEN THE RATCHET & A GEAR BOX, BREAKING THE FINGER.

EE WAS WORKING ON A MOTOR AND HAD LEFT HAND RESTING ON A PLATFORM THAT HAD AN ANGLE IRON ON EDGE. A WORKER FROM ANOTHER COMPANY WAS CLIMBING ON EQUIPMENT LADDER TO THE LEFT OF EE AND AS OTHER EE WAS CLIMBING UP THE LADDER, SLIPPED AND LANDED ON HAND SMASHING MIDDLE FINGER

A LARGE PRY BAR SLIPPED SMASHING HIS FINGER BETWEEN PRY BAR AND A ROCK; LACERATION OF LEFT LITTLE FINGER.

SWINGING A SLEDGE HAMMER HELPING REPAIR DAMAGED FRONT END LOADER STEPS, HE MISSED AND HIT HIS LEFT INNER THIGH. HE DAMAGED MUSCLES IN LEFT THIGH.

JUMPED APPROX 4 FT FROM CONVEYER TO GROUND, FRACTURE TO LOWER RIGHT LEG.

EE WAS ATTEMPTING TO START A RECOIL START AIR COMPRESSOR WHEN THE RECOIL CORD BROKE. HE FELL OUTOF THE TRUCK IN WHICH THE COMPRESSOR WAS LOCATEDINJURING HIS RIBS, BACK AND NECK (BRUISED).
