Employee was working on a dredge pipe, pipe moved causing injury to Left index finger and Thumb

Employee was fueling an excavator, employee slipped off of excavator track, striking ground injuring left shoulder.

Employee placed Left Middle and Ring fingers in a pinch point area while Shields were being lowered causing injury to employee's fingers, employee could not explain why employee placed them in a pinch point area.

Employee was using hand tool to dislodge material and EE's left pinkie finger was injured.

Employee was carrying buckets of stone for stemming holes on a stone shot when a lower back strain occurred.

Climbed out of equipment. When reaching the ground turned to walk and the ground was covered in snow and ice and stepped into a rut and rolled ankle.

Employee was installing new tracks on a excavator driving a steel pin with a sledge hammer to hold tracks together when a very small piece of steel fragment broke off and went into his rigt leg. Employee received 4 stitches in his right leg.

Employee went to jumped off Linkbelt 460 Excavator and slipped on the end of the track. Employee fell on the track with his lower back injuring it.

Employee was cleaning the grizzly plate on the feeder of the crusher with a spud bar. His hand had slipped off the spud bar and the spud bar slipped out of his other hand and smashed his little finger against the steel grizzly plate causing it to smash open. Employee went to Hospital and received 4 stitches and NO broken bones.

Employee was trying to straighten a roof bolt that was slightly bent using the step on the Fletcher Roof Bolter when the bolt rotated and slipped while in the step causing the employee to hit his left wrist on the step causing it to have a small slight fracture to his wrist.

Roof fall appears to have originated at or near the face in heading 1 & extended approx 100 to 120' outby. It is approx 30-35' in width at its widest point & extends above the roof line approx 15-19'. It is believed that a combination of low cover, lower structural quality of roof rock & possible localized horizontal stress had created the conditions leading to the failure.

EE was on knees moving parts from the floor to a steel parts bin. As he moved from a left to right position he smashed his right finger against the steel bin and the part he was holding. Which resulted in 4 stitches on his finger.

Employee was going to cut off old hydraulic tank off of frame of old control trailer when the heat got to the tank and a flash fire occurred, when gases heated up inside of the tank and vented. EE had minor 1st degree burn to face & lips. He was wearing the proper PPE and the oil splattered on the glasses.

He was installing brakes on water truck. When lifting brake drum, his hip popped out of place.

Victim was standing between the lube truck & Bobcat (skid steer) and another employee got in the lube truck to move it. Victim thought he was going to pull straight ahead, instead he turned sharp left, and caught victim's left leg by the bumper of lube truck & knocked him in the side of the Bobcat.

He finished drilling a hole and was backing drill on an even bench, the right track went into a ditch and the drill tipped over. the mast was not centered, which caused more weight to the right side.

GOT OUT OF TRACK HOE TO RELIEVE HIMSELF AND HE SLIPPED ON THE METAL TRACK PADS AND FELL TO THE GROUND.

HE WORKED 10 1/2 HOURS ON 10-22-03. HE TOOK OFF WORK ON 10-23-03. HE TOOK A PERSONAL DAY ON 10-25-03. I CALLED HIM ON 10-27-03 TO SEE WHY HE WAS NOT WORKING. HE STATED THAT ON 10-23-03 HE COUL D NOT GET OUT OF BED, DUE TO A SORE BACK. HE STATED THAT HE DID NOT KNOW WHAT HAPPENED TO HIS BACK.

HE STATED THAT THE A-FRAME BELT WAS CURLED UP UNDER THE CHUTE. HE HAD CO-WORKER TURN BELT ON/OFF TO SEE IF IT WOULD UNCURL, WHILE DOING SO, ROCKS START FLYING. HE THREW HIS HANDS UP TO COVER F ACE. HIS GLOVE ON RIGHT HAND HIT BELT CLAMP AND CAUGHT HIS HAND.

UNDER EX 270 HOE USING AN IMPACT WRENCH WHEN METAL FLEW INTO HIS LEFT EYE. DID NOT GO TO DOCTOR UNTIL 1/17/01. HAD FACE SHIELD ON.

ON 4-3-00, EE WAS PULLING ON A WRENCH WHEN HE FELT A PAIN IN HIS RIGHT SHOULDER. THIS WAS NOT REPORTABLE AT THAT TIME. THEN ON 11-27-00 THIS BECAME REPORTABLE LOST TIME ACCIDENT, WHEN EE HAD A N OPERATION.

HE WAS TIGHTENING A BOLT WITH A RACHET AND SOCKET. THE SOCKET SLIPPED OFF NUT AND RACHET FELL AND HIT HIM IN FRONT UPPER TOOTH. CHIPPED TOOTH.

WHILE HE WAS CLEANING HIS SAFETY GLASSES A SMALL PIECE OF ROCK FLEW AND HIT HIM IN LEFT EYE.

HE WAS CLEANING MATERIAL FROM UNDER AND ADJUSTMENT PIPE ON OUTSIDE OF CRUSHER, WHEN THE BLOCKS HOLDING THE PIPE FELL OVER, ALLOWING THE PIPE TO DROP ON HIS LEFT INDEX FINGER.

WHILE GETTING OFF OF DION DOZER, SLIPPED ON STEP AND FELL ON TRACK AND HIT INSIDE OF RT KNEE ON TRACK.

DRAINING CONTAINTED FUEL IN 35GAL TANK, LIDE OPENED STEPED ON LID CAUSED IT TO SLIDE STRIKING FEET MADE EE LOSE BALANCE FALLING OVER SIDE OF TANK STRIKING CONTAINMENT AREA AND THEN THE GROUN D

CUTTING BELT AND SLIPPED OFF AND CUT LEG WITH KNIFE.

SLIPPED GETTING ON TRACK HOE AND FELL BACKWARDS ON SOME ROCKS.

FELL FROM PAYHAULER AND FRACTURED RIBS AND POSSIBLE BROKEN BONE IN SHOULDER

WAS USING A 9 INCH GRINDER-GRINDER CAUGHT ON STEEL CUT THREW GLOVE INTO HAND

FOOT SLIPPED OFF BOTTOM RUNG OF LADDER AND TWISTED HIP.
