Miner was collaring hole looked up and small rock aprox 2in fell striking miner in the mouth. Miner was wearing appropriate PPE, area was clear of visible loose rock, area was bolted from the back to the face visible scaling had been done , housekeeping was good.

Mostrando los 29Miner was bolting when small rock came down and hit his left ring finger. Miners 5-point card shows that he had scaled the area prior to performing work.
Miner complained of knee pain, it was found out that he had been having knee pain for a while. During investigation it was determined the area was free of loose rock and had a level walking surface. Miner was walking to the buggy with normal walking patterns when he went to take a step on his left foot and his knee popped out aggravating a non traumatic undetermined injury.
While changing out the long hole steel Miner grabbed a-hold of the steel while driller was rotating to remove it. The Steel became bound up at an angle and pinched his thumb between the steel and the guide slide.
While bolting barred down some loose rock and continued to bolt.Slipped on a slab that had been barred down and landed on my right elbow.
Employees was unloading bolting supplies underground ( mats ) when one that a sharp edge cut through his glove he then received a laceration. Employee was wearing all PPE's at the time
Miner was driving a bolt with 1 1/2 foot driver he got the bolt tight to the back and the machine would not come down. EE went to move the machine leg forward to remove pressure off the driver and when doing so the machine came down in EE had his foot under the crows foot, the weight of the machine pushed the crows foot through EE's met-guard boot and into his foot.
Miner was using a cutting torch when a piece of slag came off and went behind his safety glasses burning corner of left eye.
Mechanic was installing a hoist cylinder by hand when he went to lift it he felt a sharp pain in his left bicep. Per another EE there is a suspicion on steroid use from this employee. Employee did not report this incident until 5:45pm
Miner was cleaning out a conex and was pushing and pulling a cylinder aprox 200lbs, Miner has had a hernia before and failed to use S.L.A.M and correct lifting techniques.
Miner was moving a ladder underground ladder became caught on a rung and miner went to lift it out during the process of him lifting the ladder he did not use proper lifting techniques causing him to strain a muscle in his back.
Miner 3 was chucking steel with a Miner 1 when the steel shanked and Miner 1 operating jack leg lost control. Miner 3 was struck in the jaw and cracked two teeth. Small scratches on jaw.
Moving jack-leg drill from one area to another, crows foot got caught on nine wire that was used to hold jack-leg to rib, lost grip on jack-leg and grabbed a hold of muffler to keep it from falling when his finger got caught in the pinch point. Lacerated left ring finger requiring stitches.
Miner was drilling the last knee hole in preparation for loading a round, when the air spud on the jack leg came loose and hit him in the knee. The whip check was properly installed on air line. Miner was on one knee next to connection. A deep contusion was the result of spud hitting Miners knee.
Drove mucker bucket down to the work area. Miner ran over a big rock by first muck bay. After he parked and got off mucker the miner bent down to pick up water hose he felt a shooting pain down his lower back then down his left butt cheek.
While bolting the back, miners drill bit came off the end of his steel and landed in a puddle beyond the last row of bolts. The miner went past the last row of bolts to retrieve the drill bit and was hit with rock that fell out of the back.
Miner was walking across travel way towards office when she tripped over a small rock. She hit and lacerated her left knee.
While maneuvering a toggle plate into place the miner felt a pain in his back and when he stood up his right testicle had a sharp pain.
Employee did brake check on surface, stoppped twice on ramp with no problems, when got to the 650 ramp brakes failed and miner hit rib to stop. Miner was not wearing seat belt and was thrown around in cab.
While removing the cribbing from under a haul truck the mechanic was stacking the wood on the side when he smashed his right hand ring and middle finger between two pieces of cribbing.
While taking down a welded wire bulk head miner leaned ladder on bulk head and gut the chain holding it up. The bulk head fell and the miner received cuts and punctures to the left arm.
EE was in the process of removing a wooden ladder from the top-mounted ladder truck of a pick up truck. EE stepped off of the running board to the ground (approx. 12") and twisted her right ankle.
Miner was stepping off of mantrip using three point contact and while planting his right leg, EE aggravated an old injury by twisting his knee.
While drilling uppers in the 220 stope, miner looked up at his drill steel and hole when a rock fell from the drill hole area and struck him in the nose and mouth.
While loading jack leg drills into a mucker bucket for removal from area the drill he was handling slipped and pinched his pinkey finger of the left hand against the bucket lip. EE pulled his hand back and ripped the tip of his pinkey off into his glove.
Put split set in one hole of the roof mat. Scaling bar that was holding mat up slipped and mat hit a rock that dislodged it. Rock fell down hitting miner in the lip and arm causing a laceration on both.
While doing ground control with a jack leg, the steel became plugged while removing the steel from hole it got stuck in. Grabbed the jack leg and when the jack leg came loose, employees finger was in a pinch point, required stitches.
While driving a haul truck the front of the box hit a 6" pipe at the vic coupler and seperated it. While exiting the vehicle miner received a laceration on his head and abrasions on his arms.
EE was hanging welded wire to shot crete the back for ground control when a sharp rock fell thru the screen striking EE on the right cheek at the corner of his mouth. EE stated he was fine and stopped the bleeding with a butterfly band-aid. After EE's shift He went to the local Urgent Care for evaluation and received 2 stitches.
