EE was moving a large cap stone, by standing it upright and walking it to the pallet. EE tripped or stumbled and fell. As EE fell the stone smashed EE's thumb between the stone EE was moving and one that was on the pallet.

EE was stacking a pallet of flagstone onto a pallet and a piece that he was handling broke in two. As a result, ee lost his grip and the broken piece came down and smashed his right middle finger. EE received a laceration and crush injury to his finger requiring sutures and a finger brace.Carmelo was wearing all of the required safety equipment including gloves.

EE was standing next to an exterior metal storage cabinet when a very strong gust of wind blew the door open. The edge of the door struck ee on the right anterior portion of his cranium, resulting in a laceration.

As EE was carrying stones over to a pallet that was on level ground, he tripped and fell forward. When he fell forward, he dropped the stone in front of him and used his hands to catch his fall. When EE extended his arms, his right forearm scraped the edge of the stone he was carrying. As the result, he sustained a laceration to the inside of his right forearm.

A rock caught his Left pinky finger between the rock and a steel bucket used for transporting rock while loading the bucket. He went to the hospital for an examination and then returned the next morning to work. The Dr. gave restrictions but he has not lost any days of work.

Large rock fell on left foot just below ankle. Was wearing steel toed boots but rock hit side of foot pinching between rock and steel track. Tried calling MSHA person for questions on 3/19/09 and received answers on 3/31/09 due to phone tag and serious illness in reporters family.

EE reported an object in his l. eye. He said that it happened on 12/29/08 when asked. He said that something had blown into it & he had been trying to wash it out for a couple days. He also said he was wearing safety glasses at the time. He went to the hosp. to have it looked at & the Dr. didn't find anything wrong at that time.

Employee was moving rock on cutting table, when rock started to fall from table. He tried to catch the rock to stop it from falling off the table, when he smashed the tip of his right hand index finger.

Worker was moving a rock when it slipped and smashed the end of his finger.

Smashed end of finger between rock and machine table.

Smashed middle finger on left hand, between rock and conveyor roller, resulting in broken finger tip.

A rock rolled and the employee tried to stop the movement and in the process sprained his right thumb.

Employee was splitting rock using a hammer and chisel. He swung and missed the chisel hitting his left hand.

Employee was stacking rock and smashed his right hand causing a contusion

Employee got hand caught under a rock causing the left pinky finger to bruise and swell.

Employee has been experiencing pain in his left shoulder from repetitive lifting. When lifting stone the pain has been increasing as of late and this repetitive pain caused him to see the doctor

Employee was lifting a rock and he strained his lower back

Employee was hammering rock with a chisel and missed the chisel smashing his left hand and fracturing it

Employee was exiting the forklift and missed his footing and fell from the step, bashing his face into the tire of the forklift, breaking his nose.

Employee was driving the forklift and a foreign substance (either dust or a rock) entered his right eye causing a corneal abrasion.

EE WAS LIFTING STONE WITH ANOTHER EE AND HIS HAND WAS MASHED BETWEEN 2 ROCKS FRACTURING THE EE'S RIGHT FINGER.

Employee was moving a rock and upon setting it down the rock smashed his middle finger on his right hand leaving him with a laceration.

Employee was setting a rock down when his finger got smashed.

Employee was working with rock and smashed his left hand

Employee was working with rock and it smashed the inside of his right hand between his thumb and index finger

Employee was moving rock and it fell from his grip, smashing his right hand ring finger

Employee was working and accidentally walked into the fork lift forks and hit his rib section. Pain progressed and did not lessen and medical care was sought.

Employee was extracting rock in the quarry when he started experiencing back pain in his lower back.

Employee was pulling piping off of a truck and dropped a piece on his right foot, breaking his big toe.

The employee was lifting a piece of rock and suffered a thoracic muscle strain.

This employee was taken to the emergency room on 07/14/04 with complaints of pain in his leg and foot. After X-rays and scans of his lower region the doctor found that he has ruptured discs in his back. He could not pinpoint an exact date as to when the injury was incurred nor could he attribute the injury specifically to any one work-related task.
