The crew was moving an 80' wheeled conveyor to the side of the pile. The conveyor was pushed with a skid steer causing it to roll, it went over the chalk block. EE tried to stop it by holding the tire back with EE's body, EE's foot slipped and EE fell into the path of the wheel. It ran over EE's upper leg.

EE WAS WALKING DOWN AN EMBANKMENT & SLIPPED & FELL HITTING THE BACK OF HIS HEAD ON THE GROUND HE BLACKED OUT FOR SHORT TIME HE WENT BACK TO WORK AFTER REGAINING CONSCIOUSNESS
