THE WALKING, WORKING SURFACE WAS EVEN, CLEAN AND CLEAR OF ANY OBSTRUCTION. SUPERVISOR, NOTIFIED LOCAL AMBULANCE SERVICE TO COME TO LOCATION FOR CARE.
EE was loading up a trax running gear for a John Deere Dozer so it could be taken to shop for repairs. EE pinched the end of middle finger on left hand. The end of finger and fingernail was smashed. EE was transported to Providence Hospital for treatment.
EE was carrying a screen (4' by 6') up the stairs to the plant and strained a muscle in the middle of his back. He stated "I do not feel like I need to go to the doctor." He called in sick on the morning of 07-27-07 and stated he felt he needed to go to the doctor at this time to get his back checked. He worked all day 7-25-07 and 07-26-07.
A CUSTOMER TRUCK DRIVER GOT OUT OF HIS TRUCK WHILE A MINE FRONT END LOADER OPERATOR WAS LOADING HIS TRUCK WITH SAND. HE WAS WORKING ON SOMETHING ON THE BACK OF HIS TRUCK AND FORGOT TO SET HIS PARKING BRAKE. THE TRUCK BEGAN TO ROLL BACK AND HE COULD NOT GET OUT OF THE WAY, RESULTING IN THE CUSTOMER BEING PINNED UNDER HIS TRUCK
