Employee was finishing up the right side of his cut while loading a buggy when he felt something hit him in the leg causing a laceration to his lower left leg.

A mechanic was removing an old U-joint from a coal truck. In the process of striking the object to punch the U-joint out, the mechanic delivered a glancing blow, which caused a 6 inch long x 5/8 inch thick bolt to fly out and hit the injured party in the mouth. This caused a cut to the mouth, one tooth to be knocked out, one tooth broken & two teeth to be loosened.
A rock fall occurred above anchorage at crosscut 14 between #2 and #3 return entries along the #1 belt at spad #176 The fall knocked out a return stopping. No one was injured or trapped, no damage to equipment. The return air course was rerouted.
An area near a previous roof fall had been cribbed the rock shifted causing the cribbing to fall. No damage was done and travel and ventilation was not affected.
The area of the fall had been bolted with 4' glue bolts, a rider seam 5' to 6' high let loose allowing a section of roof to fall. Several cribs were set in the area.

Putting up 8 ft bolts when bolt got hung up on drill steel pinchers. The up pressure was hit in an attempt to get the plate unhung. Holding on to the plate, the bolt bent, pinching his finger against the drill steel pincher; requiring 12 stitches.
While doing preshift duties, ee stepped in a hole. Later in the shift while shoveling coal, he felt a pain in his waist running down his leg. Pain was minor but noticeable. Did not miss any work until April 8th when pain became more intense. He went to doctor who advised an MRI and time off.
A roof fall occurred at crosscut 19 between #5 and #6 entries. The fall measured 5'H x 20'L x 19'W. There were no injuries but a stopping was destroyed interupting ventilation. The area had been bolted with 4' bolts.
Injured was installing a yellow rope cable hanger. Injured jumped up & hooked the hanger hook over the roof bolt plate & when he came down he caught the last 3 fingers of his right hand in a loop of rope tearing skin off end of pinky finger & bruising the other two fingers. The man was left hanging by his fingers & coworker had to lift him up to get fingers loose.
EE & other were running water thru the spray plaster machine to flush it out & the nozzle plugged up. When the nozzle cleared itself, the hose jumped out of work hand & sprayed him in the face. The force knocked his glasses off & got material in both eyes. Transported to hospital,had eyes flushed & given antibiotic.
Injured and co-worker were attempting to clear a rock from under the foot jack of the TRS. Co-worker was prying on rock with a fiberglass slabber bar and injured was trying to move the rock with his hands. The bar broke and the injured's middle finger on his left hand was caught between the rock and the foot jack causing a fracture.
Injured had pushed supplies off the scoop with the ram. He had a chain hooked around the supplies & anchored to a hook on the ram. The chain was too tight to unhook & he was holding on to the chain with his right hand & he stepped onto the chain attempting to get slack & he caught his finger between the chain & the hook cutting his finger & causing a distal fracture.
Injured and co worker were walking thu the crosscut from #4 to #3 entry. Employee stated he was looking at the roof and he stepped into an offset in the bottom causing him to fall to the bottom spraining his right knee.
A roof fall was found at 8:45am in #7 entry one break inby spad # 3418. The fall measured 4' H x 20' L x 10' W. The area had been bolted with 4' resin bolts. There was no equipment damaged or injuries but the intake had to be re-routed around the fall.
A roof fall was found in #2 entry at crosscut #19 near spad #206. The fall measured 20'X20'X4-5'high. No injuries or equipment damaged as a result of the fall. Area had been bolted with 4' resin bolts.
A roof fall was found in #7 entry on No 1 Mains between crosscut 21 and 22. There were no injuries or equipment damage. The fall measured 40'L x 20'W x 5'H. The area had been bolted with 4' resin bolts.
Employee stated he was stuffing rags into air conditioner when his finger hit the fan.
A section of roof measuring 20' w x 10'l x 8' thick fell at the highwall over #4 intake fan entry. The rock damaged the fan housing, interrupted ventilation and blocked the primary escapeway.
Injured and co-worker were rehanging line curtain in #3 entry when injured bent down to pick up the curtain, a piece of rock measuring 3 1/2' x 5' x 2" fell from between the roof bolts striking injured in the back between the shoulder blades causing hairline fractures of 3 vertebrae. Injured was treated and released from the hospital.
EE was talking to supervisor and while bent over due to seam height EE's hard hat scrapped the mine roof causing rock dust to fall behind his safety glasses. The rock dust struck him in the eye. EE has not missed any days worked~medical treatment onlu using prescription mediciation for treatment of the material in the eyes.
A roof fall was discovered when the foreman was doing his weekly airways. The fall occurred in panel No. 6 and ran from #4 ehtry through the crosscut into #5 entry outby spad #1975. The fall measured 80'L x 20'W x 6'H. There were no injuries or equipment damage. The area had been bolted with 42" resin bolts.
