Found roof fall on the 1A road at 8121' to 8041' of the north entries. Roof fall is 90' to 95' long, 17-18' wide, 7 to 8' high. Also it fell across 1A belt roadway. Had additional support previously installed (timber and cribs). Primary roof support was 6' tension rebar. Fall will not be cleaned up.

Roof fall was found by examiner at start of shift fall is at 4841-4921 between got 61 x cut on main north travelway fall is 65 to 70' long 18 1/2' wide 8-9' high fall is in entry between intersecitons primary roof support was 6' double rock with glue intersection at 4841 had been resupported with 12' cable bolts fall will not be cleaned up.
Examiner found roof fall at the 1360' in x-cut between #5 & #4 entry of the 2nd east entry along the 1A belt. Fall is 35 - 40' long, 17 - 18' wide, 7 - 8' high. Employee was replacing guard on belt at hoist. Guard was resting on bolt head when it slipped, falling down and cutting right little finger on hand.

Examiner was making weekly examination of return entry and found roof fall in #8 entry between the 1940' + 2020' X cut was in entry between the intersections. Fall is 60' long, 6' to 7' high and 17' to 18' long. Area has been timbered off. Fall will not be cleaned up. Primary roof support was 6' tension rebar.
EE was bolting at face. He was in process of putting bolt in hole and pushing bolt up into glue, when power was lost on pinner. EE then pulled bolt out of hole and struck finger on drill pot when bolt came out of hole.
Employee was using water line on unit to water haulage roads. When pulling on waterline, it hung on a rock and pulled him backwards, twisting his knee. At end of shift when riding out in mantrip, a spare tire on mantrip caught on a roof bolt causing mantrip to stop suddenly and employee bruised his tailbone.

Employee was riding out in mantrip at the end of shift. There was a piece of belt in roadway. When mantrip drove over belt the belt caught his foot pulling leg sideways.

Employee was using a cutting torch to remove a broken leaf spring on a golf cart. He was holding spring with left hand and moved cutting torch over left hand. Employee was wearing gloves and cutting goggles.
Xcut north at 1920' on 1A road (roof fall) intersection on inby & outby end of fall had previously been cable bolted roof fall 6' high 17-18' wide 35-40' long area that fell was bolted with 5' double lock bolts.
A GOLF CART BROKE DOWN & WAS SENT TO SURFACE TO BE REPAIRED. AN ENDLOADER WAS USED TO PICK CART OFF LOWBOY - WHEN LOAD WAS PICKED UP IT WAS NOT CENTERED. THE MINE FOREMAN STOOD ON END OF CART TO BALANCE LOAD. WHEN LOAD WAS MOVED, CART SHIFTED ON FORK OF ENDLOADR CAUSING CART TO FLIP MINE FOREMAN IN AIR AND FALL TO GROUND ON HIS BACK.
EE WAS ATTEMPTING TO OPEN A WALK-THRU MAN DOOR GOING INTO INTAKE AIR COURSE. STRAINED LOWER BACK.
HAD A ROOF FALL ON THE 2ND NORTH BELT @ +4601 INTERSECTION ON #4 ENTRY NORTH BELT LINE WAS TIMBERED UP TO THIS AREA. APPROACHES ARE TIMBERED FALL WILL BE CLEANED UP. BELT & STRUCTURE AND WATER LINE UNDER FALL. FALL IS APPROX 40'L, 18' W AND 7-9' HIGH. AREA HAD BEEN BOLTD WITH 5' TENSION BOLTS.
EE was using a cutting torch and cutting the bolts out of a cyclone in prep plant cyclone was suspended by a chain hoist. When EE hit cyclone with hammerto knock bolts out of the cyclone it broke free & swung around striking EE on left forearm.
EE is a belt maintenance man. As he was watching belt run, he saw a roofbolt laying on the belt. He reached to pull bolt off the belt and his right middle finger got caught in between the moving belt and roller.
EE was installing rof bolts at Face # 3 entry was getting drill steels out of drill chuck. He was going to turn on the rotation but pushed the wrong lever & drill pot went up and pinned his left index finger to the roof.
The fall is in the #8 & #7 entries of the return 2nd east entry. The fall was in #7 4 way intersection & extended across to #8 entry. The fall will not be cleaned up. In progress of setting timbers and cribbing approaches to fall.
EMPLOYEE STATED HE WAS DRIVING GOLF CART BETWEEN COAL HAULER & RIB. A COAL HAULER TIRE WAS LEANING AGAINST THE RIB WHEN EE DROVE THE CART BY RIB. THE CART STRUCK THE TIRE LEANING AGAINST RIB, AS IT WAS FALLING OVER EE REACHED OUT TO CATCH THE TIRE PULLING HIS LEFT ARM BACKWARDS.
EMPLOYEE WAS LIFTING BELT STRUCTURE FROM THE MINE FLOOR & LOADING STRUCTURE ON SUPPLY CAR.
AT ABOUT 8:00 AM, 2 GOLF CARTS WERE PUT ON CHARGE AT SHIFT CHANGE. AT ABOUT 9:55 AM, 2 MINERS SMELLED SMOKE ON TRAVEL WAY. THE MINE FOREMAN WAS NOTIFIED & TRAVELED TO SLOPE BOTTOM & FOUND 2 GOLF CARTS ON FIRE. THE FIRE WAS PUT OUT ABOUT 10:15 AM. CAUSE OF FIRE IS UNDETERMINED (INVESTIGATION ONGOING). ALL ELECTRICAL CIRCUITS SUPPLYING POWER TO GOLF CARTS FUNCTIONED OK.
The injured person was walking across face area and stepped in hole and twisted body.
In the #5 entry north supply road at 5721 to 5801 in pillar line was developed about 8/10/04. The roof was pinned with 5' tension rebar bolt. the fall was approx. 44' long 16' wide, 8' high.
The injured is a roof bolter, he was throwing rockdust on the rib, the upper of machine was setting over, while doing this he ran over the injured EE, right foot, the injured had his back to machine.
THE FALL WAS IN THE #6 NORTH ENTRY (SUPPLY ROAD) AT +5321 TO +5241 IN PILLAR LINE. WAS DEVELOPED APPROX 8/2/04. THE ROOF WAS PINNED WITH 5' TENSION REBAR BOLTS. THE FALL WAS APPROX 50' LONG, 18' WIDE, 5' HIGH.
Employee said he had put Ram battery on charge & when he jerked on battery lid his back started hurting, he worked until 11/30/05, went to Dr.
HAD FALL IN INTERSECTION, BELT ENTRY, 2 X-CUTS INBY FEEDER ON #2 UNIT OF 650'WEST PANEL. FALL MEASURED APPROX. 49' X 42' X 6'HIGH. PULLED 4' DOUBLE LOCK ROOF BOLTS. FALL IS BEING CLEANED UP.
The injured said he was getting out of cab on 488 scoop when he put his foot on the ground he heard a pop in his right knee.
THE INJURED WAS BUILDING A BRATTICE ON #1 UNIT. HE DID NOT HAVE THE CURTAIN DOWN AND NO FLAG WAS HANGING TO CURTAIN CAR DRIVER. A RAM CAR RAN THRU CURTAIN, HIT THE TRAILER OF BLOCK, HITTING THE INJURED ON LEFT LEG. THE CAR DRIVER DID SOUND HORN AND FLASH HIS LIGHT BEFORE GOING THRU CURTAIN.
The injured was drilling hole in roof to install roof bolt. While drilling hole, a burr on the steel caught his glove and fractured his left ring finger.
The injured said he was picking up a 5' long 6X6' timber when he felt a sharp pain in his right groin area.
THE INJURED SAID HE WAS HELPING MOVE BELT UP. HE WAS LIFTING BOTTOM BELT ROLLERS UP AND HANGING IT ON ROPE, WHEN HIS BACK STARTED HURTING.
The fall was int he #2 (N. intake) entry, which blocked escapeway from the #1 unit no work will be done on #1 unit until escapeway is sate to travel. The fall was 70' long, 5' high, 18' wide. The fall will be cleaned up before work is rejumed on the affected #1 unit.
THE FALL IN NORTH SUPPLY ROAD, #5 ENTRY + 5241 DEVELOPED APPROX. 10/01/04. THE TOP WAS SUPPORTED WITH 5'TENSION REBAR BOLTS. THE FALL WAS APPROX. 70'LONG X 18'WIDE & 5'HIGH. THE #6 NEUTRAL ENTRY WAS CLEANED & USED FOR A NEW SUPPLY ROAD. THE FALL WILL BE CLEANED UP. THE APPROACH TO THE FALL HAS BEEN REINFORCED, CRIBBED & TIMBERED.
The injured is a roof bolter, he said he had a bolt stuck in the hole in roof when he pulled the bolt out of roof he mashed his litte finger between the bolt head & the wrench on bolter.
Had roof fall on the 2nd M. North belt line, at +4681, intersection #4 entry. Belt line was timbered up to this area, approaches are timbered. Fall will be cleaned up. Belt, structure and water line, under fall. The fall is appx 70' long, 4 to 6' high, 18' wide, had been supported with 5' tension roof bolts.
THE 5 HP MOTOR ON PRESSURE TANK WENT TO GROUND. HAD TO CHANGE MOTOR. THIS HELPS OPERATE BRAKES. THE MEN UNDERGROUND WERE NOTIFIED & PRODUCTION WAS STOPPED. THE EMERGENCY HOIST IN SHAFT WAS SENT TO BOTTOM. THE HOIST WAS RUNNING AGAIN AT 11:00 AM. IT WAS DOWN 1 HR 45 MIN.
THE INJURED SAID HE WAS WALKING OVER TO GET THE SCOOP WHEN HE RAN INTO A ROOF BOLT, FELL DOWN TO HIS KNEES, AND HEARD SOMETHING POP IN RIGHT KNEE. HIS KNEE WAS NOT HURTING THAT BAD. HE FINISHED WORKING THE REST OF SHIFT.
THE INJURED WAS HOOKING UP BATTERY TRACTOR TO THE WATER WAGON & BRUISED HIS RIGHT HAND WHILE PUTTING THE PIN IN TONGUE.
The injured said he had drilled a hole in the mine roof, to install a roof bolt when the top section of drill steel hung in the roof - The drill steel then fell out of the roof, striking him on the left hand.
THE INJURED TOOK RAM CAR TO BATTERY BARN TO CHANGE BATTERIES. WHEN HE WAS GETTING OFF OF THE CAB OF RAM CAR, HE SLIPPED, FELL & HURT HIS LOWER BACK.
The injured took the ram car out to the battery barn to change the batteries, he said he tripped and fell down and injured his right wrist.
