Tail Pulley on conveyor was plugged and stopped the conveyor. Conveyor was locked out and gear box brake was set to prevent the belting from moving backwards. As material was removed relieving pressure, belting moved forward pulling miner into tail pulley.
Employee was shoveling off the under belt to do some repair work. Lead man went to help and check on employee. Employee was acting funny, confused, did not know location and was sweating. It was 62 Deg F. Lead man called an ambulance and the safety manager.
Employee was lowering light plant, crank handle was loose, got wrench to tighten bolt on handle, wrench slipped off bolt head, employee felt pop in right shoulder and pain.
Placing guard on conveyor after replacing drive belts, guard slipped and the employee attempted to catch the guard, guard cut through glove and R ring finger, this required 7 stitches.
Moving return rollers, pulled muscle in back.
MECHANIC AND HELPER WERE LOWERING 1/2 OF JAW INTO PLACE WHEN COME-A-LONG FREE SPOOLED, LOWER CORNER OF JAW CAUGHT AND TOP OF JAW FLIPPED AROUND HITTING MECHANIC IN THE LEGS.
EE WAS DRILLING HOLE IN PLATE FROM SCRAPER AND DRILL CAUGHT TURNING PLATE INTO POST AND CAUGHT FINGER BETWEEN POST AND PLATE.
