Employee was working to unplug belly belt on secondary crusher. Employee was positioned on conveyor that belly belt dumps onto. Employee was using a pipe wrench on head pulley to roll the belt forward to free it from being stuck. The pipe wrench slipped off the head pulley and employee fell backward landing on the ground resulting in 5 broken ribs.
Employee was attempting to pump the pit using a 3 inch water pump. Employee turned pump on and believed it was operating correctly. After returning to the pump 30-40 minutes later employee realized the pump was not sucking. Employee shut the pump off & released the hose causing hot water to be sprayed on the employee. Employee received 2nd degree burns on arms & hands.
After changing tire, employee lifted the tire into the back of the truck and in doing so injured intestine. Employee finished shift, but advised that employee thought maybe had pulled a muscle in stomach. After the shift, employee went to the hospital. They were pumping stomach to avoid surgery, but doctor advised that employee will need surgery on small tear on intestine.
EE was entering operation shack of dredge when a gust of wind blew operator into shack. EE caught self on the door frame with EE's left hand when the door shut pinching the index finger on EE's left hand. After EE's shift, EE went to Prowers Medical Center to get EE's finger cleaned up. The tip to EE's finger was amputated at the time of the incident.
Conveyor was blocked with boulders. Employee went through LOTO procedures. Employee removed head pulley guard to release tension in belt with pipe wrench. As employee was moving pipe wrench towards self, EE's hand slipped off wrench and into the pulley as it began to rotate back to original position.
Employee was climbing fixed ladder on wash plant. Employee did not have 3 points and slipped. Employee fell and landed on EE's right foot. Employee ended up breaking EE's heel.
Employee climbed surge bin to adjust gate flow. During process of adjusting the nut, employee fell approximately 8'. Employee was not wearing fall protection. A ladder and man lift were also available for use. Employee has been trained on fall protection.
Employee was changing a screen without a harness and fell off screen deck.
Employee was trying to change Skid-steer bucket attachment. The pins were not releasing due to buildup of frozen mud/ice. After getting bucket free, the bucket fell about 2' and landed on employee's foot behind the steel-toe protector. The result is a fracture in metatarsal bone.
Employee had a pipe wrench on the shaft pulling downward when the pipe wrench came loose smashing his finger between the wrench and the chute.
Dull box knife cut upper arm of employee while he was trying to cut deck cord.

Employee was changing haul trucks incorrectly and fell, which resulted in a puncture wound on left leg.
Employee was pulling on a jammed gate. When the gate gave way, the gate pinned his right thumb between a large bolt.
Employee was grinding leg on bin. Employee hit a gap with the grinder and the grinder smacked employee in nose. He broke this nose and was released to regular duty that day. Employee took the afternoon off and was back to work the following day.
Cleaning out impactor, rock fell on his head from the conveyor. He got out on his own and walked to his truck. He was dizzy, and managers called 911. He was taken to the hospital and kept overnight. He has not broken bones, but suffers from some spinal compression and two broken teeth. He is at home and will seek therapy.
Hydraulics on lift of dump truck failed, crashing into the truck. Broke passenger door, flying glass hit right eye and thrashed into steering wheel and back into seat. Intitial treatment that day, then admitted to the hospital on 9/22. He is currently in the hospital on IV pain meds for 5 bulging disks lower lumbar and swelling. Dump truck has been tagged out and is under repair.
As employee was dismounting his haul truck, the catwalk broke loose under his feet. He fell and broke 2 ribs, fractured his tailbone, sprained his hand and hit his forehead. The catwalk came loose during the course of the day.
While helping to lower crusher off blocks prior to moving it out of position, the crusher shifted and appeared to be tipping. IE and another employee ran from the crusher. IE fell and tripped on some rocks. He hit his chin and experienced some neck and jaw discomfort.
The seat in a new loader #01-109 does not give enough room for legs and he has an inflamtion of his hip/leg.
He was shoveling rock out from the rock crusher and either strained his back shoveling or reaching for a shovel.
Using wheat burner to unthaw dirt out of skid loader bucket and dirt or lime went into employee's eye.
