Employee said a rock struck their thumb while greasing. Was seen at hospital in and put on light duty for 3 days. When employee was questioned on 4/20/2022 about conflicting statements, employee resigned.

Injured employee was standing under Jaw Crusher another employee yelled and when injured employee moved quickly Hat fell off and a steel plate struck employee on top of head. Injured employee received 4 staples to close wound.

IW was cleaning pulley shaft in preparation of changing bearings/cum-a-long slipped off wing pulley pinching IW's middle and ring fingers against bearing support

He was backing up a D-9 Dozer, ran over a rock. He said it pulled his neck.

EE was shoveling at the end of the Finishing Screen. A rock fell, hitting him on the right shoulder. He reported it to his supervisor. He said he was ok and went back to work. On 2/15/07, one hour before work time, he called in saying he was going to the doctor because his shoulder was hurting. The doctor excused him from work for 2 days.

HE WAS CUTTING A PIECE OF SKIRT RUBBER AND THE KNIFE SLIPPED CUTTING HIS LEG.
