large rock got stuck in the jaw of the crusher. A TM attempted to use a chain to wedge between the rock and the jaw dies to break it up. As the rock and jaw pulled the chain down the TM tried to pull the chain up, resulting in smashing EE's hand between the chain and the railing causing laceration to left palm and a fracture to the 5th metacarpal

employee lifted the motor up to put it down where we could use the Crain to hoist it the rest of the way out. while lifting the motor employee felt something tear in the abdominal area. incident occurred 8/26/24 - it was reported 8/28/24 and found out EE was going to receive surgery 9/20/24 then surgery was moved up to 10/7/24
Employee was moving a conveyor belt onto a stand, upon moving the belt EE's fingers were caught in between the conveyor belt and the screen guarding. the screen went through EE's glove and cause a laceration to the right index finger. resulting in three stitches. employee was released for work with no lost time.
Employee was running on mine surface and rolled right ankle. Employee is unsure exactly what happened to cause the ankle to roll.
EMPLOYEE INSPECTING BLOW BARS ON IMPACT,ELBOW STRUCK THE BREAKER PLATES AND ROTOR COTINUED TO ROLL. THIS CAUSED BREAK IN RIGHT WRIST.
Employee was standing beside cone crusher after assisting to push some material through it with a frost bar. Another employee went around the crusher to manually open the cone further. Employee still by cone got right foot caught in pinch point. Employee suffered tendon damage and a broken heel as well as a laceration.
Placing cat walk on fuel tank to avoid falls while checking fuel levels. Cat walk slipped falling to ground before secured. Employee fell to ground with cat walk.
Employee was cutting rubber belting material with a razor blade knife. Employee was pulling knife towards self when it slipped out of the rubber material and EE hit EE's upper left leg with the blade. Employee received a laceration approximately 1 inch deep and 1 1/2 inches long. Employee received sutures to close the wound. Minimal impairment.
Miner was cutting rubber belting material with a razor blade knife. The knife slipped and the miner lacerated left forearm with the blade of the knife. The laceration required sutures and the miner returned to work.
Miner was using a sledgehammer and a punch to drift a pin out so that the tooth on an excavator bucket could be replaced. The miner hit their thumb with the sledge hammer when it glanced off of the top of the punch. The miner's right thumb suffered a fracture on the tip of it.
Employee was chipping hardened soil from the flange of a stacker inlet so that the flange could be changed. Employee was using a spud bar in a standing position above the inlet. On downward stroke spud bar hit rubber belting and bounced back up causing a small laceration above employees right eye. Employee was taken to medical clinic and received 7 sutures to close the laceration.
Employee and another employee were loading screen that were to be lifted up to the screen deck of a gravel crusher. While lifting the screen a sharp edge cut through the employees leather glove and left a laceration on left index finger on the second knuckle. Employee did receive medical treatment and four stitches to close the laceration.
Conditions contributing to the injury included snow on the ground and rocky terrain.
A miner, engaged in crusher maintenance, was cutting a rubber conveyor belt with a razor knife in order to patch it. He was holding the knife in his right hand when the knife slipped out of the rubber material and back at him. The miner stabbed the palm of his left hand with the knife. Damage was limited to a small laceration.
The employee was getting numbers off of fuel tank trailer SB25 and stepped off and rolled ankle
Was using hand to try and hold a bolt on a cutting edge to keep into place. The air gun spun the bolt and lacerated his hand. The employee was wearing gloves.
Employee was lifting a screen above his head and partially dislocated shoulder and tore a couple tendons and had to have surgery.
A pieces of flattened expanded metal slipped out of the employees hand struck him in the leg puncturing his left thigh in two places which required stitches.

WHILE EMPLOYEE WAS OPERATING A CAT SKID STEER LOADER CLEANING OUT UNDER A SCREEN DECK, THE LOADER LUNGED FORWARD AND IN AN ATTEMPT TO KEEP FROM GOING FORWARD, HE PULLED A GROIN MUSCLE. HE WAS INJURED BECAUSE HE FAILED TO WEAR A REQUIRED SEAT BELT WHILE OPERATING THE LOADER.
EE WAS ASSISTING IN TEARING DOWN 5 BIN FEEDER AND WAS REMOVING A JACK PAD. INJ EE & FELLOW EE LIFTED THE PAD SET IT ON A SHORT WALL. THE PAD TILTED AND ROTATED CATCHING EE'S RIGHT RING FINGER AND CUTTING IT AT THE 1ST KNUCKLE NEEDED 5 STITCHES
EMPLOYEE WAS ATTEMPTING TO REMOVE A TAIL ROLL OUT OF THE BACK OF A PICKUP BED & AS HE PULLED ON THE SHAFT THE SHAFT AND TAIL ROLLED MORE EASIER THAT ANTICIPATED AND THE END OF THE SHAFT CAUGHT EMPLOYEES HAND BETWEEN IT AND THE CORNER OF THE PICKUP BOX.
WORKER LABORER/OPERATOR SLIPPED ON A FROST COVERED SURFACE, AS HE STARTED TO FALLHE CAUGH HIMSELF BY PUTTING OUT HIS LEFT HAND, THIS CAUSED HIM TO BRUISE HIS WRIST.
WORKER WAS ATTEMPTING TO WALK UNDER A LOW CONVEYOR. WHILE DOING SO TWISTED HIS LEFT KNEE, SPRAINING THE KNEE.
