Spider was confirmed to be black widow by emergency care providers.
While lifting 25HP electric motor from mount the lifting eye on top of motor broke and dropped 2' hitting mount and rolling off mount onto employee's foot. Lifting eye was installed by manufacturer. Steel choker & clevis were used for rigging. Truck hoe was used for lift.
Dredge oper. was in the bottom of dredge checking the pump when he lost his footing on his right leg. When he came down on his right leg his knee was twisted or sprang.
Stepped off dredge to frozen ground. Rolled ankle over, bad sprain.

Around 8:00 pm the loader was going to the pit to put lights on. Went to shop to fuel loader bucket was bouncing. Then the loader stopped. EE had no seat belt on. He hit the windshield with his forehead and nose.
Operator was mining wall throughout the day, When about 4:20 pm operator was backing out from wall when wall collaspe. Material fell on front end of loader and hit windshield, causing windshield to break and laceration to operator left and right arm. Highwall is about 25 to 30 feet high.
Employee stepped from scales to deck to clock in and in doing so turned his foot causing pain in his lower back.
