April 16 2020 the employee had surgery on EE's wrist because EE had carpal tunnel.

A piece of rock fell and hit employee on the head. The hard hat cut employee above eye brow causing employee to get 5 stitches.

employee put a hand on the bolter and a piece of shale feel from the top and hit middle finger causing a small fracture.

Had carpal tunnel surgery on 02/11/2020.

Throwing a pallet in a dumpster when it hit a foam canister in the dumpster squirting a liquid in employee's face and eye. Employee had to get a prescription for eye.

Slipped on tram struck knee on tram.

Mantrip hit something causing it to go through the drain hole on the floor board fracturing employee's leg.

Employee had Carpal Tunnel Surgery on both wrists on 2/26/2019.

An unintentional roof fall was discovered at 1R2LMS entry #1 at maker 62+55 intersection.

Employee had carpal tunnel surgery on both wrists. Surgery is on 9/4/2018.

An unintentional roof fall was discovered by the examiner at 1R2LMS entry #7 Between station numbers 63+30 and 64+05.

Repeated motion. Went and had surgery on elbow for carpal tunnel on 07/14/2020

Miner was walking away from the miner while they were changing bits when a piece of the bit or hammer hit the employee walking away in the side of the head. The employee received 5 sutures.

Employee had Carpal Tunnel Surgery on 1/17/18.

Employee was straightening roof bolt plates when a piece of rock fell and smashed right ring finger. EE received 7 sutures and broke the bone at the end of finger

Elevator had an electrical problem

Employee was scaling the top when a piece of rock fell and hit ee in the head. EE received 15 staples.

Employee had Carpal Tunnel Surgery on wrist on 4/4/2019.

Stepped on a piece of coal and twisted ankle. The employee never missed a day of work until 11/02/2017. The ankle never really healed and now the doctor has put EE in a boot and told EE to stay off it for 2 weeks.

Truck driver dumping fly ash got dizzy fell into a pile of fly ash. Employee received burns to employee's eyes which employee had to get antibiotics for employee's eyes.
