Employee was helping a truck driver get a rock out of the tailgate of their dump truck. As employee and truck driver pulled the tailgate open, the rock fell out, truck driver let go of tailgate, the tailgate swung shut and struck the employee's right pointer finger. Employee's right pointer finger was fractured.

Mostrando los 17Employee was sitting inside of a skid steer. When the employee climbed out of the skid steer, EE slipped on the ground and twisted EE's left knee.
EE was supervising another employee who was operating a power drill. The bit on the drill broke and the broken piece ricocheted and hit EE in the front tooth. As EE yelled and put hand up to mouth the employee operating the drill pulled the drill down from where the operator was drilling and the sharp end of the bit struck EE's right hand, cutting EE's small finger.
Employee's yearly hearing test showed a standard threshold shift.
Employee was removing 4" discharge pipe from the CFS slurry pump. After removing the pipe, the pressure that had been built up inside of the pipe let loose and hit the employee in the face with sand and water. Employee suffered burns on both cheek bones and above left eye. EE has safety glasses, hard hat and ear plugs on.
Driving a ground rod when he missed with hammer causing left hand to hit the top of the ground rod resulting in a laceration.
Sprained left shoulder from carrying steel sheet.
Employee was hammering on a piece of metal clamped in a vice when his hammer came in contact with the plant structure on the down swing causing the hammer to deflect into his thumb. Laceration to left thumb requiring six sutures. Vice was relocated to an area without obstructions.
Employee was cutting rubber belting with a utility knife when it slipped and stabbed him in the left leg.
Employee was walking across parking lot when he slipped on ice and fell onto his back resulting in a contusion to his chest wall and back.
Employee was taking load of anti-skid in articulating truck to pit. It was snowing and there was a fresh coat of snow on the haul road. Employee hit a frozen pile of millings laying along the side of the haul road with the front right tire of the articulating truck he was operating at which time the cab rocked and the employee struck the right side of his head on the window.
Employee was clearing trees when a previously cut tree which was stuck in the branches of a second tree which was being cut fell and struck employee on the back. Employee suffered several spinal fractures and two fractured ribs.
Employee was installing blade guards on a P65 Dozer for transport when his little finger on his right hand was pinched between the guard and the track frame causing a laceration requiring 4 stitches.
Injured employee and another worker just finished repairing a damaged belt. Injured employee was picking up extension cords to take back to the tool trailer. When he started down the stairs he slipped and fell resulting in a ligament sprain to the right foot. The steps were not wet or slippery and there was nothing on the steps creating a tripping hazard.
The employee had just finished filling the water truck with water and was in the process of disconnecting the water hose from the truck. He took a step back and stepped on a #4 size rock. This caused him to lose his footing, resulting in a sprained Right ankle and knee.
Employee was walking by a wood work bench in the Lab when his left hand brushed the bench. From this contact the employee received a splinter in his knuckle of his Left Index finger. Employee was taken to the ER where he received 4 stitches.
Employee was cutting a 4in rubber pipe with a utility knife when the injury occurred. He was standing and holding the pipe in front of him with his left hand, the employee was using a utility knife in his right hand to cut the pipe. The knife slipped from the pipe and struck the upper portion of his left leg resulting in a laceration requiring 7 stitches.
