Employee was lifting and turning their leg when it broke. This is the employee that had previously broke their leg at their house in August 2025 and it's the same exact leg. It fractured in the same location of a previously installed screw.
A miner was fatally injured when material fell from the roof of the mine, striking the cab of the front end loader the miner was operating.
On February 20, the miner was being task trained to operate a haul truck. EE sat in the jump seat for approximately 2 roundtrips to the crusher. On February 26, miner informed management was experiencing lower back pain since riding in the haul truck.

I am a owner operator semi driver, i was dropping off a load of front loader buckets and had got to the unloading area and was proceeding to unchain the load. I was on my trailer using a bar to undo the binders so i could take the chain off and the bar flew off the end of the binder and struck me in the face. The result was a facial laceration that needed 5 stitches.
Employee was shoveling material off a platform which is below the primary screen tower when a rock fell from above and struck employee on the side of EE's hard hat.
The employee attempted to step on material at the bottom of a rib after the employee completed identifying (spray paint) the rib. Employee fell and bruised back.
Crew was pressing dies into place on jaw using a portapower. Portapower unit broke, which caused the die to swing out and roll employee's foot, which caused the steel in the steel-toe boot to pinch foot.
EE pulled off to the side of the road and exited truck 177 to add salt to the load of stone. It was 2 degrees outside and EE got ice on EE's shoe and when EE went to step down and EE fell straight down off of the step and hit the ground where EE was immobilized. 911 was called and an ambulance was sent.
A maintenance procedure was needed to be performed on the drill requiring the use of cribbing for safety. EE went under the drill to help with stacking the cribbing. While under the machine on their knees EE rotated their body to pick up a piece of cribbing to put on the stack and felt a very sharp pain in their back.
