While exiting 1394 Haul Truck, Left hand holding rail, right foot stepped down on next step but foot slipped. Landed on buttocks on the bottom of last step.
While dismounting from Excavator, foot got caught between steps. There was a dip in the ground which caused employee to lose balance while releasing hand rail. Employee fell backwards and landed on right elbow.
While lifting rocks off the Drill Pattern, strained lower back.
While operating the Skidsteer, the employee traveled over a slight incline and the skidsteer jerked, causing EE's body to jerk as well. Shortly after, EE felt tingling and slight pain in neck and back.
Miner slipped and hit knee while lubricating the crusher plant before startup.
Employee used right foot to move hose. Slight pain in right ankle. 8/9/22 appointment, return to full duty. Follow up appointment on 11/4/22, employee was placed on restrictive duty on 11/17/22
While removing an a/c line from vehicle, lost balance while pulling it from the evaporator core and fell on hand to brace self.
While cutting with the Plasma, air blew debris under safety glasses and hit left eye
While holding bearing race. A piece of the material broke and flew in left hand while another employee punched the race out.
Employee twisted right ankle while exiting Conveyor #5 walkway, stepped to ground and fell.
While loading blast holes. EE was moving side to side, left to right, and front and back. Allegedly strained left muscle calf. EE sought medical attention on 9/13/2021
While removing bolts on the top of the tail pulley guard, the guard slid down and cut into employee's left ring finger
Working on an oil tank, Crusher #3, when a hose not connected, swung and hit employees head. Area struck, swelled up to approximate size of a golf ball.
Miner was using an impact drill and hand wrench to loosen bolts on a side guard rail when EE's finger got trapped between the steel frame and wrench.
Miner was throwing a chain over a beam while standing on a step ladder. Chain slipped, yanked their hand. Miner's left hand hit the ladder forcefully resulting in a laceration of the top of left thumb.
On 3/12/21, employee reports for the first time that they have right ear pain/ringing from noise exposure for approx. 10-15 minutes when training conveyor belt in Primary Plant on **2/19/21 without hearing protection which company provides. **Employee was not at work on 2/19/21.
Employee was walking towards the tool room when they stepped off the concrete slab to the ground and twisted an ankle. Employee forgot about the slight drop off from the concrete slab to the ground.
While moving trougher on C-2, trougher slid off C-2 frame and hit 3 fingers on employee's left hand.
Employee stepped off from skytrak lift onto uneven ground and felt pain to right knee.
While walking on the catwalk between crusher 5 & 6, EE forgot that the hatch door to gain access to the crusher landing was open and fell through the opening striking left side abdomen against the catwalk opening. Access from the catwalk to the landing area is about 3' down. A miner, was down below cleaning the landing area but did not witness the fall.
Miner was doing a pre work inspection on wheel loader. Miner climbed a ladder to a platform on the rear of the loader to check fluids. Miner then climbed from the platform on to the tire of the loader, instead of going back down the ladder and attempted to get into the operator's cab and twisted a knee.
Miner was operating a wheel loader to load the primary plant crusher when they ran over a large rock. This caused the loader to slam down with tremendous force, jarring the lower back of the miner and causing pain.
Operator of 773 haul truck reported a leak in truck. As they traveled to a safe spot, the hydraulic ram on the right side of the truck wedged between right rear inner tire & frame causing tire to come off the rim. Operator in the cab alleged to have ringing in left ear from the tire blow out.
Miner was operating a 988 Cat Wheel Loader and strained right knee.
Miner was trying to lift a full tank of acetylene in to the bed of service truck when they strained lower back.
Miner came in to the shop for a lunch break and was exiting the water wagon with EE's lunch cooler in EE's hand. EE's left foot slipped off the top step of the water wagon and fell to the ground, landing on EE's left shoulder.
Miner stepped on an unsecured 2 X 4 wood brace support for a foundation precast, when it gave way causing miner to fall against the compacted sub-base embankment, striking the right side of ribs. Miner sustained right side 9th rib fracture and is on modified duty.
On 10/03/18 Miner woke up in the morning with pain in left knee. Miner believes ee strained left knee from climbing in and out of manlift basket and leaning on basket rails when working on conveyor 23 bearings. Miner reported injury on 10/03/18
While welding on the excavator bucket, Miner pulled welding gloves off, proceeded down the ladder & positioned right hand onto the bucket to balance, when the stinger fell. The stinger fell from the top of the bucket, hit side of the bucket and the air arc cutting rod burnt top of right hand. MINER SOUGHT MEDICAL TREATMENT AND REPORTED INJURY ON 9/12/18
Miner was welding plates in the reject chutes of Tower 23 and welding overhead with head tilted in a left position within a two and a half (2.5) hour period. On 9/06/2018 ee reported pain to this left side of neck.
After adjusting the Hazemag, Miner stepped onto the motor and slipped onto the working platform. EE landed on EE's right leg bearing most of EE's weight and felt immediate pain to EE's right knee.
While exiting through the hopper hatch for crusher 14, miner felt a sharp pain below the right knee cap. Miner continued to work and felt knee getting tighter. Miner reported the injury the next day at the start of the shift.
Miner was using a 2 ton chain come-a-long to jack up a shaker and change a motor mount spring, when the come-a-long chain broke. The chain struck left elbow and the block (housing) struck right forearm and right leg.
While Lifting a 35 gallon grease barrel from the ground onto the flatbed truck (#290), employee strained right shoulder. Barrel weighed approximately 80lbs.
EE saw co-worker at Screen 15 with the hood opened 3/4 of the way. The hood was blocked by screens stacked in front of it. In an attempt to lay the hood flat, EE crouched under the hood to lift/push screens out of the way and ee felt pain in lower back thereafter. Pain did not go away, EE out on injury from 3/15/17 and sought medical attention on 3/17/17.
On 11/17, while operating eq. 2754 Loader in phase 2, EE said EE could feel the twinge in the shoulder. When EE woke up on 11/18 EE said it was more sensitive. As the day progressed on 11/18 at work, the repetitive motion of manually leveling the bucket sent sharp pains up the arm into the shoulder. EE contacted supervisor at 12 pm.
While working on Tower 10, my left hand got smashed between 4' crow bar and guard rail.
While trying to jack up tank 3 leg, the 4" pipe popped out and fell onto hardhat then upper back of miner.
Employee operating 773 Haul Truck (1389) proceeded to drive down main haul road above primary plant, when they noticed another Haul Truck driving up. They immediately braked to a complete stop. Following behind employee was a 600 Komatsu loader (2799) operator. The Loader bucket hit the haul truck. Immediately after, employee felt pain to their back.
