Miner was operating a haul truck and bringing sand to the underground mine. The haul truck struck the side rib of the decline shaft. The miner suffered rib fractures as a result of the impact.

Mostrando los 7Employee was trying to free blockage of rock/material in a tail pulley using a pry bar. The pry bar slipped and the employee fractured EE's right ring finger. Equipment was de-energized and locked out when employee performed this task.
Employee dismounted the basket of the ANFO Loader and stepped on a chunk of limestone, twisting EE's left ankle.
Employee states was working with a come along when employee felt a pop in shoulder. The employee did not report the accident to supervisor. Employee stated EE pinched finger between the boom and the basket when the stop block broke off and allowed the boom to lower more than typical.
