Employee was cutting a piece of pipe with a utility knife. The blade broke they slipped forward when the blade broke and broken blade cut right wrist on a downward angle. This happened approx. 6:00PM on 07/14/2026 came to section we stopped bleeding and took outside and to ER.

While moving the car anchor, cable slack was drug under the operator's compartment. EE stood beside the deck, and started the car. The reel engaged and pulled from under the car, striking EE in the legs. It flipped EE over and threw EE to the ground. EE complained with pain in EE's left knee. EE was taken to the hospital by ambulance.

Employee was bolting in the #5 entry were they swing out to put up outside bolt when a piece of rock hit them in the back.

Miner was crimping water line with banding tool. After completeing the job, EE then pulled the excess band out of the banding tool and the sharp edge cut the thumb and finger on left hand.

EE was swinging Drill Boom on Fletcher Bolter to install the inside bolt after installing outside bolt. Piece of draw rock struck EE on the back of the right foot while crawling with boom swing.

Employee was making repair to the miner when employee slipped and fell injuring left leg when hitting the stand off of the miner

Driving a small buggy underground front tire hit a rock causing steering wheel to turn excessively fast hitting EE in the right index finger

Operator was moving from one heading to another and placed hand on safety post outside. The operator when turning the cable chain came in contact with safety post pinching left pinky finger.

Scoop operator was pulling up slack on the miner cable when cable tightened up it struck miner operator on left knee
