A helper on an underground core drill was lifting a 5' HQ drill rod to add to the drill string when they lost footing and fell to the wooden deck (floor). The drill rod fell with them and landed on right thumb causing a fracture to the thumb.

While stepping out of the portable toilet next to the dispatch office the dispatcher fell backwards striking back of neck and head. Physician advised dispatcher to remain off work for 2 to 4 weeks.
Miner was using mine phone when they were struck in the back by a sheave block which broke free from the bolt/utility plate it was connected to. Miner received soft tissue injuries only.
While stepping off bus employee felt pain in right knee.
IW was in the process of pulling out a core tube. IW was holding the latch head of the tube while the other helper was disconnecting the overshot. Once the overshot was released from the tube, the tube fell back down into the rods. IW did not let go of the latch head fast enough and left thumb and right middle finger both suffered lacerations that required sutures.
Employee was remote mining with a 2 yard loader and articulated the loader on top of their foot, badly bruising it.
A raise miner pulled on the air and water line which had become stuck. Miner repositioned and pulled hard on the lines and felt a pop and a burning sensation in groin area. Miner reported the incident and maintained was OK. Miner was taken to the doctor where it was determined miner had a hernia, recommended miner see a surgeon to determine treatment.
dismounting from loader and hit the ROPS with EE's hard hat knocking dirt loose and into EE's eyes (hard hat and safety glasses were on)
Mine lost power 6/11/17 at 4:45 AM & all personnel were evacuated. On 6/12/17 an electrician entered to check pump with gas detector (natural airflow of 55K CFM). 1280 ft. down the decline the EE encountered a rib fall on left side with material impeding travel, returned to sur., notified mngt. at 9:45 AM. During msha investigation est. 50-75 tons blocking 25% of decline
Employee was installing a split set bolt with a jackleg and pushing bolt in with a 2' driver. The chuck tender on drill came open and the driver came out of the drill and fell down striking employee on the right ring finger which caused a fracture to the finger.
