Cutter AGG driver (NOT Heidelberg employee) proceeded to the pit loading area and exited the truck to correct an issue with the gate locking system. While attempting to engage the gate latch manually, the latch moved unexpectedly, causing the gate to slam shut and crush the end of the driver's middle finger. The driver immediately left the facility to seek medical attention.

Mostrando los 18Employee was using an angle grinder kneeling on the ground grinding a piece of steel. the grinder had guard in place and a handle. when finished they went to get up with the grinder in right hand the grinder still had some momentum and struck left hand index finger. They called the supervisor and went to urgent care and received 8 stiches. Employee was wearing C.R. gloves
IE and a Peterson CAT Mechanic were torquing bolts on a 980G at the wheel ends. On the last bolt when EE went to torque down there was a miss communication and IE had IE's middle finger on IE's right hand in the way and it got smashed on the downward movement of the torque wrench.
Employee was working in the de watering screen and when they went to lift up a water sprayer it caught on floor level and an abrupt stop happened. They felt no injury but later that night around 7pm they started to have some pain.
Employee was operating loader and skidsteer and felt pain in their back. Later was diagnosed with a shoulder strain/tendonitis.
Employee was using an air chisel to remove a broken bolt and a piece of shrapnel came out from the area of work and got lodged into the worker's left forearm about an inch deep. The air tool was inspected by a service company and it was determined that debris did not come out of the tool.
During a screen change, employee was standing on middle deck screen. EE put foot on the top deck crown bar to brace self while pulling the screen off the forks. While pulling, EE's foot slipped and EE hit shin on the crown bar. This caused a laceration that required 8 stitches to close.
Rim side ring was heavier than employee anticipated.
Mechanics were performing maintenance on a Cat 375 excavator. The two mechanics set the part on the body of the excavator to adjust the rigging. While unhooking the rigging from the crane, the swivel began to roll off of the side of the excavator and caught the synthetic strap moving it towards the mechanic's hands. The strap caught the employee's finger, bending it backwards.
Employee grabbed a 1/2" steel plate from a work bench. When EE slid it towards self, EE's thumb got caught between the table and the steel plate. Employees finger was crushed and sustained injuries requiring medical treatment (stitches).
Haul truck operator runs over a pot hole and bruises ribs on arm rest of operators seat.
Possible workplace transmission of COVID. Employee #1 was being in person trained for the scale attendant position. Employee #1 received a call from a friend that is COVID positive. Employee #2 was identified as a close contact (per CDC definition) and was quarantined. COVID testing took place and employee #2 was COVID positive.
Two employees were replacing return rollers on the #3 pit conveyor. When dropping one end of the roller in place an employee felt themselves overextend left arm causing pain in shoulder area. Employee worked the remainder of the shift without further incident. Employee elected self care until decided to be seen at a clinic 03/22/2021 and received physical therapy.
Employee was operating a skid steer at the tailings pile when ee ran over a 4x8 rock causing the skid steer to lurch, jarring back.
Slipped and fell injuring back.
Grease on handrail, causing employee to slip and fall. Broken wrist.
At lewisville Pit EE was using a grinder with a cut off disk to cut the head off of a bolt. The grinder dug in causing the grinder to spin in EE's hands. The cut off disc came into contact with EE's thumb causing a deep laceration. Another EE is taking EE to the clinic and will be updating as we know more. EE has been with the company for 6 years.
Employee was changing a bearing on a screen. While lifting the bearing, EE felt a pop in EE's back. EE went to urgent care and was diagnosed with minor lower back strain.
