Laborer was changing V-belt on Screen. Screen was locked out. They didn't loosen the the take up enough and tried to roll the belt on the pulley and got a finger pinched requiring stitches in their right ring finger tip.

Employee's radio fell off EE's belt but the mic was still attached to EE's collar. the cord pulled the stretched and retracted the radio. the radio flew back and broke the employee's tooth.
Loader operator fell off the deck of a parked front-end loader receiving injuries to ribs, sternum and bruised left lung.
Employee received flash burns to their eyes while wielding. The PPE they were wearing was insufficient to protect their eyes. They experienced discomfort to their eyes and was treated for minor eye irritation at the ER and released back to work the same day.
Employee was standing inside of wash plan sand screw installing a chute. the employee lost footing and stumbled backwards. They reached up to grab something to stop them from falling. They grabbed the chute they were working on and pulled it down on top of their leg. It resulted in a laceration that required sutures. Employee was treated and released with no restrictions.
Employee split EE's lip with a chipping bar.
Employee cut their thumb with a razor knife.
Employee was lifting a pump part after disassembly and it slipped out of EE's left hand pinching EE's right index finger between the part and an edge on the pump housing. Treatment required finger surgery.
Employee climbed onto conveyor to make a repair without following lock-out tag-out procedures and when conveyor started EE fell off injuring left forearm.
