Employee was using a piece of rebar in an attempt to dislodge a rock that was wedged between the conveyor frame and the head pulley of the VSI crusher feed conveyor. In doing this, the rebar got between the head pulley and the belt while it was running. This caused the rebar to strike both of EE's hands resulting in the amputation of the left thumb and the tip of the right thumb.

Mostrando los 4Office employee fainted, due to an ongoing personal medical issue, and fell striking right shoulder on the ground. This resulted in a dislocated shoulder and small fracture to one of the bones in the shoulder.
(IW) was adjusting the crusher apron with shims on the portable crushing unit. A hydraulic ram that raises the aprons had a bent cylinder, which limited the stroke. The preshift inspection sheet did not indicate an issue nor was it reported to the Manager. IW installed a makeshift support whereas the tip of EE's thumb caught between the cylinder and the makeshift support
The employee lost footing on the last rung of the ladder while dismounting dragline, and fell to the ground. Recommendation: utilize a ridged mounted flat bar step in lieu of swinging chain and bar stock. Re instruct employee in safely mounting and dismounting equipment utilizing the 3 point method.
