Employee was stacking 60 inch top roller clusters on a pile when EE lost EE's grip on the roller and it fell out of EE's hand and smashed EE's right pinky finger. 5 sutures were required to close the wound.
Employee was walking in the #5 entry of 3rd East Mains between XC 6-7 when foot slipped and EE fell to the ground. As EE fell to the ground, EE reached out with left hand to catch the fall, resulting in an injury to EE's left thumb. X-rays indicated a fractured thumb.
A fire was identified out-by the 7th South #2 seal at approximately 9:58 PM. Efforts to extinguish the fire were unsuccessful, All miners were evacuated. There were no injuries to any miners.
While walking up the longwall face at approximately shield # 112, the employee tripped and lost balance and hit head on the shield leg.
Employee was operating the left side of the roof bolter located in the #2 entry face of 12th North, when their foot slipped and they fell hitting right knee and left wrist on the operators platform that they was standing on.
Employee unhooked a chain that was holding up a bundle of wire mesh roof screens that were leaning against the rib in the 12th north section. The bundle of screens fell over and hit the employee and knocked EE to the ground.
While attempting to attach the fork attachment to the mounting plate on a Ghel skid steer in the 10th North Section, the forks fell off of the attachment mount and landed on employees left foot.
Employee was stacking used belt structure onto a flat bed trailer. The stack of rollers started to tip over and as the employee reached with right hand, a roller smashed right hand against the bed of the trailer resulting in a laceration to the hand which required stitches.
Employee was kneeling down on shield #2 on the 9th North longwall face preparing to push the headgate drive. As they pushed the drive, their left foot was pinched between the relay bar and the shield toe.
While standing at shield #125 on the 9th North longwall face, the bret-by cable flipped out of the cable tray and struck the employee, thus knocking EE to the ground.
While standing on shield #124 on the 9th North longwall face, the bret-by cable flipped out of the cable tray and stuck the employee, thus knocking the employee to the ground.
The employee was getting out of the cab of a shuttle car at crosscut 37 in the 4th East Mains Development section, when EE's foot slipped on the mine floor, causing EE to fall to the mine floor. As EE fell EE tried to catch EE's fall with EE's left hand, resulting in a fracture to EE's left wrist.
Employee was unloading 60" belt structure off of a flat bed belt structure trailer located at the mouth of the 12 North section. As employee stepped off of the trailer, employee sprained right ankle.
The employee was attempting to mount a PMCR bracket onto shield number 142 on the 9th North Longwall set-up face when the PMCR bracket slipped and smashed middle and ring finger on left hand, resulting in a laceration which required 2 stitches.
Employee was removing a Jennmar Metal stopping in the 9th North set-up face when EE's foot slipped on the damp floor resulting in the employee sustaining a right ankle sprain.
Employee was loading shield to shield cables and power supply boxes at the 8th North extraction face into a tub, when a power supply box slipped and smashed EE's left thumb.
The employee was moving water hoses out of the walkway along the 8th North Longwall monorail. Employee was on EE's knees and as EE reached up to grab ahold of a monorail dolly to pull self up, the dolly moved and smashed EE's left ring and middle fingers.
Employee was cable bolting in the 11th North section. EE was pushing the drill steel into the hole and bottomed out the hole. The drill steel bowed out and broke and hit EE on the left side of their face resulting in a laceration and a fractured left cheek bone.
Employee was mudding a stopping at crosscut 12 in the 4th East Mains when EE slipped and fell. EE's left arm hit the pump skid resulting in a laceration which required stitches.
While loading coal with the miner in the #1 face inby crosscut #2 in the 11th North development section, the employee was struck by a lump of coal on the right hand, resulting in a laceration to EE's right middle finger that required stitches.
Employee was attempting to loosen a 1" water hose coupler by using 2 pair of channel locks. When the coupler broke loose, the employee felt a sharp pain in the right wrist.
The Employee was standing on a ladder to place a cap piece on top of a temporary roof support when a piece of coal fell from the face and struck the ladder and employee. Employee fell to the ground and the piece of coal landed on right leg.
The employee was removing the bucket attachment from the Gehl Skid Steer, and when EE released the attachment keeper pin, the bucket fell off of the attachment plate and slid into the the right knee. Resulting in a contusion to EE's knee.
The contract miner was assisting another employee to hang a vent tube in the #4 entry in the 4th East Mains Section. As the two employees lifted the vent tube above their heads, they lost control of the vent tube and as the contract employee attempted to regain control of the vent tube EE strained EE's right shoulder.
While attempting to guide shield #8 up the hill, the employee was using a cap piece to extend the base steering ram cylinder and when pressure was applied, the cap piece broke and smashed the employees left ring finger requiring stitches.
Employee was operating the right side drill of the roof bolter and as employee was positioning the drill steel and adjusting the angle of the drill box, employee's self rescuer pouch inadvertently snagged the rotation lever while employee's left hand was on the drill steel.
Employee was walking down stairs and reported EE tripped on the stairs and felt a pain in EE's back.
As the employee was walking along the side of a parked 650 shield hauler, employee stepped on a rock and twisted right ankle. The shield hauler was parked in the intake entry at the mouth of the 7th North section.
The employee was in the process of dropping a vent tube from the mine roof when the employee's foot slipped in the mud. As the employee fell to the ground, employee injured left lower leg. The result was a hairline fracture to left fibula.
Employee had just finished checking the shuttle car anchor and as they turned to walk away they slipped and lost balance resulting in spraining right ankle.
Employee was stacking metal roof mats onto the roof bolter machine. EE had lifted one end of 2 mats and placed it onto the machine and then walked to the other end of the mats and lifted them off of the ground and was sliding them onto the stack of mats, when EE slipped on the damp mine floor and jammed their right index finger.
While walking in the 9th North belt entry, the employee tripped over a belt tailpiece anchor pin and and landed on left knee resulting in knee damage that required surgery.
Employee was shoveling the ribs in the 4th East Mains when a piece of coal fell from the rib and struck the employee on top of hard hat and left foot. Employee was diagnosed to have sustained contusion and neck strain.
