An EE was in the process of cutting a vertical wall cut using a Sky stone wire saw. The saw had been running at normal startup amps. During this time the employee walked behind the saw at the exact time the diamond wire failed and struck the employee on EE's right leg. Causing a laceration to the mid thigh. After physician consultation amputation near the knee was completed.

Miner was adding a section of drill steel using a wrench to tighten the connection. The miner's left thumb was caught between the drill boom and the wrench causing a small avulsion to the left thumb.
