Employee was forcibly attaching - pushing - one end of a new air hose onto a fitting attached to the drill. While pushing, he felt a "pop" in his right elbow, with pain traveling to his right shoulder.
A conveyor belt split in half. During the repair, a wedge was used to lift a hopper. When the wedge was knocked out it struck the employee on the right cheek causing a 1.5 cm laceration.

EE performs daily equipment repairs. He alleges his back has been bothering him off and on at times (he weight-lifts at a gym every morning) but while at work he lifted a hammer and he felt a low back pain. He notified his supervisor-who then sent him to see a Doctor. It was determined that he has a herniated disc-so he decided he wants it to be surgically repaired.
While employee was working on repairing a conveyor belt he hit himself in the abdomen with a pry bar. He will need surgical repair of his ruptured hernia.

EE WAS ASSESING THE CONDITION OF THE CONVEYER, COMING OUT OF THE TUNNEL. HE TURNED TO GO AND SHUT THE LINE DOWN, AND TO GO GET THE TOOLS HE NEEDED FOR HIS REPAIRS. WHILE TURNING HE GOT TOO CLO SE AND HIS GLOVED HAND WENT UP AND OVER THE BRACKET WHICH CAUGHT HIS FINGERS BETWEEN A GUIDE ROLLER AND THE BELT.
EE IS A LOADER OPERATOR. DURING A BREAK HE WAS OFF HIS LOADER WALKING ALONG THE ROADWAY, HE OBSERVED A BOULDER THAT HE THOUGHT MIGHT CAUSE SOMEONE A PROBLEM AND TRIED TO PUSH IT TO THE SIDE OF THE ROADWAY. WHILE ATTEMPTING TO MOVE THE ROCK HE RUPTURED HIS BICEP.
WHILE PERFORMING WINTER REPAIRS ON EQUIPMENT, HE SLIPPED AND FELL. STONE CHIPS HAD BEEN SPREAD AROUND THE WORK AREA, BUT DUE TO EXCESSIVE MOISTURE IN AREA, GROUND AND BOTTOM OF WORK BOOTS WERE AFFECTED.
WHILE WORKING WITH A CAT MAINTENANCE PERSON TO CHANGE TEETH ON THE BUCKET OF THE LOADER, HE DROPPED A TOOTH OF THE BUCKET ON HIS FOOT. HE WAS WEARING STEEL TOED SAFETY BOOTS AT THE TIME OF THI S INCIDENT.
