Cleaning chute on conveyor; employee started to step down and shoe lace became caught in conveyor belt resulting in laceration to upper left thigh.
Employee was operating front-end loader lifting a jaw dye & moving it from yard area to jaw crusher on the haul road. As he made a 90 degree turn on haul road with a slope, loader turned over onto its right side resulting in bruises to right ribs/lung.

EE WAS PICKING UP SCRAP METAL FROM BACK OF FRANKLIN QUARRY SHOP AND TOSSING IT INTO LOADER BUCKET. ONE PIECE HAD A HOOK-SHAPED END ON IT AND AS HE TOSSED IT INTO LOADER BUCKET, THE HOOK-SHAPED END CAUGHT HIS LEFT WRIST.
Employee was setting up a portable water pump to fill water truck at the quarry. While hooking couplings from hose to pump, he twisted his upper right arm muscle causing a strain.
EE WAS REPAIRING A 330 TRACKHOE BUCKET IN THE SHOP. B0TTOM WAS CUT OUT AND WERE GOING TO INSTALL NEW BOTTOM. WHILE GRINDING OFF UPPER LEFT HAND CORNER OF BUCKET WITH GRINDER GRINDING WHEEL GOT CAUGHT IN CORNER OF BUCKET CASUING GRINDER TO KICK BACK AND STRIKE LEFT INDEX FINGER.
Ee was operating skid loader. he was starting to exit the skid loader and placed his right foot on the bucket. his foot slipped off the bucket and became pinned between the bucket and lock. he lost his balance and fell forward causing disolcation to three toes on his right foot.
Employee was tightening crusher when a 3 inch rock came off high screen and bounced across crusher and hit left wrist causing bruise.
EE WAS OPERATING A TRACKHOE, SCALING AND CLEANING THE HIGHWALL, WHEN HE WAS STRUCK BY A FALLING ROCK FROM ABOVE HIS WORK AREA.
EMPLOYEE WAS USING TORCH TO CUT BOLTS ON ROLLER FRAME. HE REMOVED HIS GLOVES TO PERFORM ANOTHER TASK & WHEN HE WENT BACK TO THE ROLLER FRAME IT WAS STILL HOT & HE BURNED HIS LEFT MIDDLE FINGER

EE WAS REMOVING DRIVE LINE FROM FUEL TRUCK AND FOREIGN OBJECT DIRT/RUST GOT INTO HIS EYE.
EE WAS USING A SLEDGEHAMMER ON THE CRUSHER RUN BIN AND HE TWISTED HIS KNEE.
EE ON HAUL ROAD; RAN OVER A DIP ON ROAD CAUSING SCRAPER TO BOUNCE & BOWL TO BOTTOM OUT STOPPING SCRAPER. THE FORCE OF THE SCRAPER STOPPING CAUSED EE TO GO FORWARD & HIT AGAINST STEERING WHEEL OF SCRAPER. WHEN HE HIT STEERING WHEEL IT THEN KNOCKED HIM OUT OF CAB OF SCRAPER. EE STATED HE BUCKLED HIS SEATBELT BUT EVIDENTLY IT WAS NOT SNAPPED IN THE LOCKED POSITION.
EE WAS HELPING TO INSTALL NEW TEETH ON TRACKHOE BUCKET USING A SLEDGEHAMMER AND A PIECE OF METAL STRUCK EE'S CHEEK.
EMPLOYEE WAS REMOVING ROCKS FROM CRUSHER; PICKING UP A ROCK AND PULLED MUSCLE IN LEFT ARM.
ROCK CAME OFF OF TUNNEL BELT.
WALKING ON PLANT YARD, STEPPED ON ROCK, TENDONITIS - RIGHT KNEE.
CHANGING SCREENS/MUSCLE STRAIN/RIBS-LEFT SIDE.
EMPLOYEE'S FOOT SLIPPED AND FELL FROM BUMPER OF MECHANIC'S TRUCK.
EE HAD BEEN SHOVELING USING A HAND TOOL.
LIFTING LEGS OF CONVEYOR FRAME WHEN HOOK SLIPPED CAUSING LEG ASSEMBLY TO FALL.
