Truck backed into the bin got loaded. Left the bin and the arm stuck back. So EE went over to look at why the arm was stuck. EE stood in front of the arm to look at situation. Co-worker yelled at EE to get out of there. EE turn to walk away and the arm dislodged on it's own, hitting EE in the back and throwing EE in the air.

Employee was placing the sharp edge of a loader bucket and it shifted pinching the flesh of his left long finger.

Miner was removing damaged hood from haul truck. After removing pins from hinges, miner was removing tension rod with hammer when it came loose and sprang up and made contact with miner' s hand.

Ran over rock size of basketball with loader, had loosened seat belt 2" - 3" due to heat. bounced around in cab of loader, continued to work until 7/3/2013.

Moving too fast as I was climbing ladder and slipped on step.

Slipped off step of truck and fell on his elbow. Small fracture occurred to bone near elbow. Restricted activities using one hand.

He got a CAT980 loader stuck and jumped from the platform of the loader to the surface rather than using the ladder on the loader. The jump injured his ankle.

Great River Material EE was standing next to his endloader at the time of blasting of a rock ledge. A piece of fly rock from the blast struck his right leg at knee level, breaking his leg.

Conditions contributing to the accident are unknown. Victim has broken ribs, broken collar bone, and a collapsed lung.

The primary crusher was shut down due to a boulder. Our plant man/mech had positioned himself to open chamber door, he did so before impeller had come to a complete stop. Then thinking he could stop the impeller, he put his hand into the pinch point between impeller and door opening.

Two employees were pullig a truck tire off the rim. It came off faster than expected and the rim fell on one of the employees left foot.
