Truck backed into the bin got loaded. Left the bin and the arm stuck back. So EE went over to look at why the arm was stuck. EE stood in front of the arm to look at situation. Co-worker yelled at EE to get out of there. EE turn to walk away and the arm dislodged on it's own, hitting EE in the back and throwing EE in the air.

Great River Material EE was standing next to his endloader at the time of blasting of a rock ledge. A piece of fly rock from the blast struck his right leg at knee level, breaking his leg.
Conditions contributing to the accident are unknown. Victim has broken ribs, broken collar bone, and a collapsed lung.
The primary crusher was shut down due to a boulder. Our plant man/mech had positioned himself to open chamber door, he did so before impeller had come to a complete stop. Then thinking he could stop the impeller, he put his hand into the pinch point between impeller and door opening.
Two employees were pullig a truck tire off the rim. It came off faster than expected and the rim fell on one of the employees left foot.
