Employee was walking upstairs to breakroom and slipped resulting in their left knee twisting/buckling.

The employee was clearing a material jam from the jaw crusher. EE lost EE's footing and fell onto the deck with EE's ankle bent beneath EE.

Employee slipped on ice, fell and struck EE's head on the ground. Initial complaints were of pain in EE's head and right shoulder. Several days later, EE still didn't feel well and went to the ER. Possible head injury, will be off work at least 5 days.

On 9/27/24, employee was tightening 5/8" bolts with two wrenches when EE felt a sharp pain in the wrist. EE's wrist and hand became swollen. An x-ray revealed EE had a small piece of metal embedded in their hand from a scratch EE received the day before. Medical professionals removed the metal from EE's hand. EE's wrist was only strained.

Employee was using a hammer to remove an old bearing from a crusher shaft. A sliver of steel broke off and entered the employee's leg. An x-ray showed the metal inside employee's leg, doctors opted not to remove it at this time.

Employee was dismantling a piece of equipment on the floor of the shop, EE stuck the back of EE's hand on a nearby wooden pallet and received an 1 1/2" long sliver in the back of EE's hand. Medical personnel cut the skin to push the sliver through to remove it.

The employee was asked to help pull down on a pry bar positioned overhead. The pry bar slipped and struck the employee in the head. Employee was not wearing hard hat.

Employee was removing the rear panel from a screen, lowering it with a chain come-along. The panel rotated and pinned the employee's leg against a nearby pipe.

Changing screen and finger got pinched between bolt lock and side plate.

Truck driver forgot to latch the tailgate before being loaded. EE tried to clear stones from the latch by hand. EE's fingers got pinched by the tailgate and lacerated by the latch.

Company electrician was lifting a 40lb roll of wire into a van from the ground and injured their shoulder. Electrician did not lose time at the time of the incident, but needed surgery to repair the injury on 11/28/2018.

Employee was bending over to put a steel cover on a pontoon when EE felt a sharp pain in EE's lower back.

Employee was shoveling snow when EE slipped on ice, fell and hit EE's head on the ground.

Employee was using a 115-ton Ironworker to cut miters on steel angles. During the job, the dies shifted until they came in contact with each other, causing a shard to break off the top die, which struck the employee in the left side of neck, requiring surgery to remove it.

Employee was using a utility knife to cut conveyor belting. The knife slipped and cut the employee's finger. Surgical glue was required to close the wound.

Two employees were hand-loading a fuel pump into the bed of a pick up truck. As they set the unit down, One employee had finger crushed by the pump base. EE had surgery to repair the finger on 11/17 and returned to work on 11/21. EE is currently on restricted duty.

Employee was stripping a belt to be vulcanized. He was using a knife to cut the cords and cut his thumb.

Employee was turning a 4-sided die on the press brake. One end of the die slipped off the support chain and crushed the employee's finger.

Two employees were loading items onto a trailer. Weather conditions were dry and very windy. Wind blew grit into one employee's eye which became embedded.

Employee was welding, repairing a rock screen. When he finished, he lifted his helmet to check his work. A piece of slag popped off and flew up under his safety glasses into his eye.

employee was stepping up onto the step on an excavator when he felt a pop and tear in his left calf.

Employee was lathing bushings in a loader bucket. A large metal shaving was stuck to the boring bar and the employee grabbed it with his gloved hand to break it off and cut his finger. The laceration required stitches to close.

Employees were removing steel plate and rebar from a pallet that was raised on a forklift. The supervisor instructed the forklift operator to lower the pallet. Instead, the operator moved the forklift back as the supervisor was removing a piece of rebar. The rebar dislodged a piece of plate which fell onto the assisting employee's foot.

On 8/27/12 the employee was cut on his elbow when the nut he was trying to loosen broke free & his arm hit the frame of a machine. He received 1st aid & returned to work. On 8/28/12, while using a wrench to turn a thread-tapper & compensating for the pain in his elbow, the wrench slipped, glanced off his safety glasses & struck him in the forehead, causing another laceration.

Two employees were shoveling trash into the back of a garbage truck. One employee lifted his shovel as the other was leaning down and struck him in the forehead with the shovel.

Employee picked up a can of welding rods. A sharp edge on the can caused a cut to the employee's thumb.

Employee was attaching a chain to a piece of equipment, preparing to move it. He pinched his thumb between a pulley and the lifting chain.

EE was repairing a dredge bucket. He stepped onto the teeth of the bucket, where he twisted his ankle, causing a hairline fracture.

EMPLOYEE WAS DISASSEMBLING A CRANE BOOM WHEN THE SUPPORT SECTION FELL OFF OF THE BOOM, ONTO THE EMPLOYEE'S FEET.

EE WAS GRINDING A PIECE OF METAL WHILE WEARING SAFETY GLASSES. A PIECE OF SLAG FLEW INTO HIS EYE.

EMPLOYEE WAS SWINING A 10-16 LB SLEDGEHAMMER IN A BENT OVER POSITION WHILE HELPING TO CHANGE A BEARING ON THE WASH PLANT.

EE WAS HELPING MOVE TOWERS WITH OVERHEAD CRANE IN FABRICATION SHOP. LOAD ON CRANE WAS NOT EQUAL& ABOUT 200LBS OF THE STEEL TOWER WAS BROUGHT DOWN ON LEFT FOOT. WENT TO ER AFTER WORK CRUSHED ARCH AND STRETCHED LIGAMENTS

EE WAS TIGHTENING A BOLT ON STEADMAN CRUSHER.

EE WAS WORKING ON A HAUL TRUCK WHEN STEEL HYDRAULIC SUSPENSION LINE BROKE. PRESSURE CAUSED OIL TO HIT RT FOREARM CAUSING ABRASION & HEMOTOMA TO IT AND SPRAYING OIL ON CLOTHING.

EMPLOYEE WAS WORKING ON ROOF OF NEW METAL BUILDING. EMPLOYEE SLIPPED, FALLING FORWARD AND HE HIT HIS NOSE.

EMPLOYEE WAS WORKING ON ROOF OF SHOP. HE SLIPPED, FELL AND CUT HIS FINGER.

EMLOYE ESTEPPED OUT OF A CABOVER TRUCK HE WAS DOING MAINTENANCE ON. HE BRUISED HIS RIGHT HEEL.

INSTALLING A DRIVE CHAIN ON DRYER AT BLACKTOP PLANT.

EMPLOYEE WAS PERFORMING ENGINE WORK ON TK#84, LIFTING OFF HEADS. INJURY DUE TO STRETCHING AND LIFTING.

EMPLOYEE WAS PUTTING SCRAP METAL INTO A DUMPSTER ACROSS THE ROAD. WHEN HE GOT BACK TO THE SHOP HIS BACK GOT PROGRESSIVELY SORER.

EE WAS ATTEMPTING TO REMOVE FROZEN CHUTE FROM PLANT. WHEN CHUTE BROKE LOOSE IT PINCHED RING FINGER BETWEEN CHUTE AND SCREEN. EE WAS WEARING GLOVES, SO EXTENT OF INJURY WAS NOT VISIBLE. WE TOOK HIM TO HOSPITAL. THEY MOVED HIM TO ANOTHER HOSPITAL WHERE FINGER WAS REMOVED.

EE WAS ACID WASHING A DUMP TRAILER. ACID WASH SPLASHED INTO EE S RIGHT EYE CAUSING A BURN TO RETINA. EE WAS USING SAFETY EQUIPMENT INCLUDING RAIN SUIT SAFETY GLASSES AND RUBBER GLOVES.

JACK FELL ON EE'S RT MIDDLE FINGER.

CUTTING A PIECE OF LUMBER, GOT FOREIGN SUBSTANCE PRESUMABLY A WOOD CHIP IN RIGHT EYE.

EMPLOYEE WAS GRINDING A PIECE OF STEEL IN SHOP. PIECE OF RUST WENT INTO EYE - NEEDED TO BE REMOVED WITH TWEEZERS.

EMPLOYEE WAS DISASSEMBLING A HYDRAULIC CYLINDER AND WORKING NEAR ANOTHER EMPLOYEE WHO WAS HAMMERING ON A STEEL TRUCK FRAM. THE NOIS OF THE HAMMERING APPARENTLY CAUSED NERVE DAMAGE TO THE FIRST EMPLOYEE'S RIGHT EAR.

WHILE WORKING IN SHOP--PIECE OF STEEL WENT IN EYE. EYE WAS FLUSHED OUT.

WHEN WELDING A PIECE OF FLUX FLEN INTO EMPLOYEE'S EYE. EMPLOYEE DID NOT SEEK MEDICAL TREATMENT UNTIL 2/16. DOCTOR TREATED INJURY, PUT PATCH ON EYE, EMPLOYEE MISSED WORK ON THAT DATE, RETURNED TO WORK ON 2/17.

EMPLOYEE TWISTED RIGHT KNEE X RAYS SHOW TORN CARTILAGE.

CARRYING A STEEL DOOR, HIS LEG SIMPLY BROKE.
