Mostrando los 7While loosening the fill plug on the final drive of an excavator, the hot drive oil sprayed on the employees right hand resulting in burns that required medical treatment.
Employee experienced chest pains, due to a personal underlying and pre-existing heart condition. Employee was transported to hospital where EE was diagnosed as experiencing a heart attack. Employee underwent stent procedure to treat arterial blockage. Employee remained responsive and alert at all times. CPR was not performed. AED was not used. Employee is back to work full duty.
Had been raining earlier that morning and ladder steps were wet.
Readjusting plant level, hand slipped causing him to fall forward and his middle finger went into a hole, resulting in him loosing approx 3/8" from his middle finger.
CHAIN WAS HOOKED TO STEEL FRAME WITH BACKHOE. CHAIN WAS HOOKED TO FRAME INSTEAD OF WRAPPING AROUND TO CONNECT TOGETHER. CHAIN SLIPPED WITH HIS WEIGHT, KNOCKING HIM DOWN HITTING HIS LEFT LEG AN D CATCHING HIS RIGHT ARM BETWEEN THE HOOK FRAME AND FRAME ALREADY ON THE GROUND. FRAME FELL APPROX. 8 INCHES FROM GROUND.
EMP WAS REMOVING LUGBOLTS FROM GEAR BOX ON CONVEYOR REACHED UP WITH HAND TO GET UP & APPARENTLY PULLED ON GEAR BOX
